Went over safety standards and plugging operations. Next went to the well to begin plugging. The first action was to set up a safety perimeter and a first and second muster area. Specialized Safety set up monitors and alarms and then allowed the plugging crew to add Sulfide blocker to the annular space between the 8 5/8 inch casing and the 4 1/2 casing. After the blocker was applied the readings dropped to 1 or 2 PPM. Nicks Well then inserted a spear into the 1 1/2 inch diameter tubing and was able to pull out one joint. The rabbit came up and they think the tubing is clear. Appalachian Well should be able to run in and shoot off the seating nipple at the bottom. They are scheduled for Tuesday morning at 7 AM |

1460645743 | 2016-04-14 | Production Wells | Violations Noted | I am on site accompanied by Inspector Mark Moreland, I witnessed the production tanks have been removed and production equipment consist of a horizontal separator. The well is utilizing a plunger lift system equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, and 1.5 inch diameter production tubing. All valves are in the closed position. Utilizing an ODNR Honeywell PhD6 meter I recorded readings of 100 ppm H2S at the well head. The well head has lost integrity and is venting H2S (Hydrogen Sulfide) gas to atmosphere through a quarter sized hole placed on the side of the well head. It appears all strings of casing/tubing have also been impacted by the poisonous gas, causing the loss of integrity and promoting higher safety/environmental risk. The well has not produced commercially since 2005. After we conducted our inspection, I then placed a call to Pat Ervin (Atlas Nobles Supervisor) at (330) 319-3155 to inform him that this well is presenting an eminent threat to the environment and health/safety. I also stated that the well needs to be plugged as soon as possible. Pat informed me that he would provide me with a written plan and he understood the well was a hazard and needed to be plugged. Pat also stated that he plans to get the well plugged as soon as possible. The well owner/ operator shall apply for a permit to plug the well. The well owner/operator shall submit a written plugging schedule to Joe Zollars at 2207 Reiser Ave S.E New Philadelphia, Ohio 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 60 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
