0342236602 2016-01-05 Drill / Deepen / Reopen No Violations On my inspection this day, accompanying ODNR Inspector Eric Conrad, Patterson Rig 288 was drilling 12.25 bore hole at a current depth of 6,173 feet. A walk-around inspection of the well pad indicated some deficiencies in secondary containment curbing, specifically flattened curbing at the edge if the rig matts in heavy traffic areas, which were brought to the attention of the Drill Site Manager.
0996977583 2016-01-12 Drill / Deepen / Reopen No Violations On my inspection today, Patterson Drilling Rig 288 was drilling the vertical section at a current depth of 9,781 feet and anticipate starting their Kick-0ff Point for the horizontal section at approximately 9,875 feet. Their anticipated Total Measured Depth is 19,640 feet. A walk-around inspection of the well pad shows that the secondary containment has been improved on the back yard side of the rig by extending liner, rig mats and curbing to include the dry chemical area all the way to the pad earthen berm, with drive-over ramps protecting the high traffic areas. Small slip on far right corner of pad slope has had additional silt fence added below the slip until better weather permits more repair. Current conditions are heavy blowing snow with 3-
1384230144 2018-03-29 Production Wells No Violations The well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit. From there, it is routed to a production area that contains six 400-barrel production fluid holding tanks. Annular pressures read on the gauges provided appeared to be 1500 pounds on the outside of the 5.5-inch diameter casing and the inside of the 9.625-inch diameter casing. 1445261668 2015-10-05 Casing String Witnessed No Violations On this day Falcon Drilling Rig # 23 had reached a total depth of 1120. The 18.625 casing was ran into a 22 open hole to a depth of 1114. Halliburton cement company was on site to cement the casing with 570 sacks of Class H cement. Cement to surface established. See cement ticket for details.
1453402405 2016-01-21 Drill / Deepen / Reopen No Violations On my inspection today, Patterson Drilling Rig 288 has reached Total Measured Depth of 19,640 feet and is nearly finished setting in place their 5.5 inch production casing. Baker Hughes is on site and is partially rigged-up waiting to cement the casing. A walk-around inspection of the well pad shows that the secondary containment is still well maintained around the rig, all auxiliary equipment, and chemicals.
1964496530 2016-02-01 Preliminary Restoration No Violations On this day this site appeared to be in satisfactory condition. Erosion and sediment controls are in place and functioning properly. Preliminary Restoration for this site has passed.
2029093151 2016-01-05 Drill / Deepen / Reopen No Violations I am accompanied by inspector Nicholson. Our inspection finds the drilling contractor, Patterson Drilling is drilling a 12.25 borehole at a depth of 6177. The anticipated total depth of this section is 9700. Deficiencies in the containment system were pointed out to the company representative as well as, an erosion channel that is forming at the south end of the pad.
2066981251 2017-06-14 Completion Testing No Violations During this inspection, the well was currently in the flow back phase of completion. There was piping from the well that lead to a sand separating unit and then to the 500 barrel fluid storage tanks. There was a total of ten 500 barrel fluid storage tanks on site. The tank battery on site contained four 400 barrel production fluid holding tanks and there were also gas processing units on site.
