1343006105 2015-12-23 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Jim Nicholson I witnessed production equipment included a sand trap, a line heater, a GP Unit along with 2-400bbl purge tanks within containment. The well was equipped with Sweco wellheads and production trees with valves in the open position with 1950psi on the tubing and 2050psi on the casing. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF.
1403652435 2014-06-24 Drill / Deepen / Reopen No Violations On the day of my inspection, I witnessed Universal Well Service break circulation with fresh water then displace four hundred five sacks of Type-1 cement with a weight of 15.6 and a yield of 1.21 down the well bore to a depth of one hundred foot, I witnessed twenty two barrels of cement returns back to the cellar.
1407719616 2014-08-10 Casing String Witnessed No Violations On the day of my inspection I witnessed that Alpha Hunter Drilling Rig #9 had drilled a 24inch diameter hole to the depth of 438 feet and ran 442 feet of 18.625 inch 87.5#/ft. J-55 casing 4 feet above KB. The last joint of casing had to be washed down. Halliburton was on site gained circulation while washing casing down with 210bbls fresh water then pumped 25bbls of gel, 10bbls of fresh water and cemented casing with 430 sacks of Class A cement and displaced cement with 122bbls of fresh water. Had good cement returns of 32bbls and cement stayed at surface.

The well was equipped with a plunger lift system with valves in the open position with 370 pounds per square inch on the tubing and 530 pounds per square inch on the casing. Fluids and gas are separated at the GP unit and then transferred to the Borton CGF.

Completed walk around with Eclipse EHS Technician Mike Hartshorn which revealed no violations.
