1196018257 2010-08-18 Drill / Deepen / Reopen No Violations At the request of Petroleum Development Corporation (PDC), an inspection of the drilling pit was conducted. The blow pit has been dug and measured approx 32x10x15. A Logan Gas Employee (ie company partnership with PDC), Mr. Ray Sinclair states that they will be enlarging the depth of this pit. Recommended to Mr. Sinclair, to be aware of north wall stabilization, due to substantial decline on hillside. The mud pit will be dug today. Logs have been placed below the well location for soil stabilization. Silt fencing will also be installed later today per contractor. Well site is acceptable and no violations were noted.

1684308003 2010-07-21 Urban Drill / Deepen / Reopen No Violations A pre-site inspection was conducted with Mr. Jim Kilbarger and a representative from Petroleum Development Corp. The drilling permit was mistakenly issued prior to the pre-site inspection. During the inspection, it was noted that the well owner began building the location. The well owner and their partner, Mr. Jim Kilbarger have been instructed to cease all activity until the revised drilling permit has been issued. The overall location at this time is rural and does contain mature trees. The fencing and landscaping/screening requirement will be waived at this time. The representatives were informed that land development and/or vandalism, they shall agree to install fencing. If the lines are routed to separate property and additional equipment added, landscaping may be required. An access gate will be required for the entance into the well site located off SR 93. Best Management Practices were recommended. Although location is rural, temporary fencing will be required when no personnel are present.

1891119578 2010-11-04 Urban Drill / Deepen / Reopen No Violations An inspection revealed service rig, but no activity at this time. The well was shut-in at time of the inspection. The well consisted of tubing, meter with chart and line routed to a flare. A PVC plug placed on bottom of 200 bbl horizontal tank (Frac tank style and not the flowback tank) contained a small leak. An earthen dike surrounds this tank therefore, no impact outside the dike. High Range Quantabs showed maximum unit of 10 ppm (mg/L)Cl-. Note: 8.8 units is equal to 6765 ppm. Notified well owner of inspection results by telephone following inspection.
