1424965167 2015-02-24 Plug / Plug Back No Violations On the date of inspection I am on site to witness the re-plugging and abandonment of the well for coal mining purposes. I am accompanied by ODNR Inspector Patrick Shreve. The well was previously plugged and abandoned through the Orphan Well Program. I was informed by Coastal Drilling representative Mark Wilson that the Coastal Drilling 220 rig had set a 16 inch diameter conductor weighing 65lbs/ft to a depth of 20 feet. I was not notified and did not witness the conductor being set. The rig then drilled to a total depth of 1239 feet below the Kelly Bushing (KB) at a height of 10 feet above ground surface. The following bit sizes were used to drill out the previously plugged well: 12.25 inch diameter from surface to 109 feet, 9.875 inch diameter hole from 109 to 698 feet, and 7.875 inch diameter from 698 to 1239 feet. No casing was present in the hole, except small, irretrievable pieces of junk metal. Coastal then completed a Gamma Well Log, Magnetic Deviation Log, and a Caliper Gamma Log. The Pittsburgh coal seam was logged from 377 to 382 feet in depth. Based on the information obtained from the logs, the following open-hole sections were calculated for cementing purposes: 1220 to 840 feet for 8 inch diameter, 840 to 780 feet for 10 inch diameter, 780 to 710 feet for 8 inch, 710 to 490 feet for 11 inch, 490 to 110 feet for 10 inch, and 110 feet to surface for 15 inch. Three plugs were set through 3.5 inch, 13.3lbs/ft drill pipe to bring cement to surface. Omega Cementing pumped 140sk of Class A cement, 13.1ppg, 1.92 yield, 8% gel. Cement was displaced with 4.4bbl of water. 20 joints of drill pipe joints averaging 31 feet were pulled. Omega pumped 160sk of Thixotropic cement, 14.7ppg, 1.27 yield, 2% Gypsum, 2% CaCl2, 2% Gel, and one gallon of yellow dye across the coal seam. Cement was displaced with 1.8bbl of water. Eleven joints of drill pipe were pulled. The cement was allowed to cure for approximately one hour then was tagged at 241 feet with drill pipe. Omega p

COAL COMPANY HAS BEEN IN CONTACT WITH DAVE BISHOP AND INFORMED HIM TO PLUG WELL TO OIL/GAS STANDARDS AND THEY WOULD PLUG TO FEDERAL STANDARDS IF AND WHEN THEY MINE THRU THE WELL SITE.
