2349251 Primary Facility 2015-03-04 Routine/Complete Inspection No Violations Noted Conventional oil well completed and appears operational.Near this well is a tank battery consisting of 4- 210 bbl stock tanks # 10354, 10355, 10352, and 10353, along with a small compressor and separator and brine tank. There is adaquate containment. No apparent leaks or spills. The location is frozen and snow covered. No violations noted at this time.
2828163 Primary Facility 2019-01-16 Routine/Complete Inspection No Violations Noted The main access road entrance for this location is off of Cobham Park Rd. in Glade Township, Warren County, PA.

This is a combination oil/gas producing well fitted with an electrically driven pump-jack. The well is plumbed to two buried gathering lines. Main valves for both lines, at the well head, appear to be open. There is no evidence of any major leaks or spills around the well head. The well location is positioned a short ways from the main accessing road, in an open area, near the top of a hill. The location and access road are currently snow covered. Vegetation, however, can be seen poking through on and around the location. E & S related BMP's seem to have been put in place upon initial construction of the accessing road and location. Regular, continued maintenance, especially with regard to the main accessing road, is recommended. The operators well ID is vaguely written on the electrical service box of the pump-jack. There is also a plastic well tag stuffed in one of the stabilizing legs of the
