| Fred Webb and Jimmy Ballew were the company men on site.  AP was tool pusher for HP.

On Friday (11-11-11) a pump plugged near the well head.  The hoses spilled there contents (oil based drilling mud) when an attempt was made to correct the situation.  The contents of the hose(s) spilled on to the pad liner.  Mr. Webb estimated that about 20 gallons of mud was spilled on the containment.  This mud was immediately vac'ed from the spill area.  No mud traveled from containment to the pad.
 
PPC plan, E&S plan and site drawings were available at the head of the access road.
 
BMPs were installed and maintained.  There continued to be steady rain at the time of inspection.  Some sedimentaion was occuring along the ditching around the pad.  Considering the precipitation, the sedimentation was minimal.  The discharge entered a rock line channel, followed by sed socks.  No sedimentation was leaving the site.
