| In response to the previous slope failures a plan (attached) was implemented to rock armor the impacted slopes and install an underdrain to convey water from the cut slope.  The previously slumped slopes have been armored and the remaining material applied and hydro-seeded.  The drain was installed, but was not conveying water at the time of the inspection.  Ironically water was ponded on the surface of the pad and slopes were visibly wet (indicating water seeping out).  At the entrance to the site, a sump was constructed and seeded.  Silt sock remains undercut and has not been effectively maintained.  In addition, the heavy rubber erosion matting/channel liner has been undercut and a channel is forming.  Please address this when it is practical as this channel will continue to contribute to E&S concerns.  Please address these E&S measures ASAP.  Overall, I am concerned for the long-term stability of this site considering the amount of water visible even now in relatively favorable (dry) conditions.
