SS 2H: TD 6692¿ with a plug from 6648¿ to 6144. IC casing to 3018¿. SS 3H: TD 6621¿, IC casing to 3007. For both wells, methane was leaking (few bubbles, slow rate) from the join between the intermediate vent pipe and the intermediate casing (Altair 5X gas meter calibrated on 1/1/23/17). Notified Greg Malekos of the leaks. For the SS 6H, a plugged conductor, all that was visible was a gravel filled cellar. For the SS 4H and SS 5H, also plugged, the cellars were filled with water. The surface hole locations for the plugged conductors were updated in eFACTS to address locational discrepancies observed in the field.

| A routine, post-plugging, inspection of the 2H and 3H original boreholes was conducted November 21, 2018. The operator has recently submitted a Certificate of Plugging for each of these original boreholes after both were plugged prior to reaching intended TD as part of a plug and skid event. At the time of inspection, the operator was on location. I inspected the wellbore, annular vents, and cellar of each well. We discussed the marker requirements on a plugged well; each well marker may say "Original borehole" to help clarify any future confusion. No obvious issues were observed at surface during this inspection.

| I met Ross Drnjevich of Inflection on the site.  H&P 521 on location, drilling the 2HN well at 8300'.  The chemical containment appeared to be in good condition, but also contained several inches of snow and ice.  The secondary containment for the mud tanks had been bolted on with wood strips on the inside of the containment in the back, approximately 4-5" down from the top.  Although it is unlikely that this brought the containment volume below 110% of the volume of the tanks, I still recommended to Ross that these liners be attached on the outside of the berms.  Meltwater had built up to 3-4" deep on the backside of the pad, and Ross told me he would have it vacced.

There was a hose in place between the water tank and the produced water tanks on site. This well is top-holed with a nightcap. Water and ice in cellar, no bubbling. 2 surface x intermediate vents, 1 conductor x surface bent. Methane was not detected. Note permiting and reporting discrepancies associated with other wells on this pad.
2554404 Primary Facility 2017-01-12 Routine/Complete Inspection No Violations Noted Stunner Smith 3H (SS 3H), 081-21279: top-holed-only with nightcap. Three annular vents (2 – 13-3/8” x 9-5/8” vents and 1 – 20” x 13-3/8” vent). Methane was not detected at any of the vents.

On 2/8/16 Inflection monitored pressure on the 9-5/8” casing. Pressure was 250 psi, and blew down in 15 minutes. There were no violations for the Stunner Smith 3H.

2686983 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted Arrived at 13:15. Weather: warm, sunny, T¿s in upper 40s. There are 7 wells. The Stunner Smith 1H and Ryder Park 1H are in production. Stunner Smith 2H and 3H are top-holed. SS 2H: TD 6692¿ with a plug from 6648¿ to 6144. IC casing to 3018¿. SS 3H: TD 6621¿, IC casing to 3007. For both wells, methane was leaking (few bubbles, slow rate) from the join between the intermediate vent pipe and the intermediate casing (Altair 5X gas meter calibrated on 1/1/23/17). Notified Greg Malekos of the leaks. For the SS 6H, a plugged conductor, all that was visible was a gravel filled cellar. For the SS 4H and SS 5H, also plugged, the cellars were filled with water. The surface hole locations for the plugged conductors were updated in eFACTS to address locational discrepancies observed in the field.

Gas isotopic sample number: 20180301237102G acquired at 11:10. SS3H: final pressure: 5-1/2 psi on 0-100psi analog gauge. Gas isotopic sample: 20180301237103G.
2806666 Primary Facility 2018-11-14 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of the 2H and 3H original boreholes was conducted November 21, 2018. The operator has recently submitted a Certificate of Plugging for each of these original boreholes after both were plugged prior to reaching intended TD as part of a plug and skid event. At the time of inspection, the operator was on location. I inspected the wellbore, annular vents, and cellar of each well. We discussed the marker requirements on a plugged well; each well marker may say "Original borehole" to help clarify any future confusion. No obvious issues were observed at surface during this inspection.

2818958 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted A H&P rig is drilling on the Smith Well pad. The Department representative did not locate the rig manager so Department could not determine which well was being drilled.

The site had a primary containment overlaid with wood plank rig matting with beveled edges (see photo). All materials held on containment and within secondary containment. The overall site conditions were clean and well organized. No leaks or spills detected. No violations.

2819626 Primary Facility 2019-04-01 Drilling/Alteration No Violations Noted Weather was 34° and sunny at time of inspection. Met with Inflection representative Greg Malekos on location. Rig was currently running in the hole with an 8 1/2" bit and 8 5/8" reamer to begin drilling the well bore for the 5 1/2" production casing. No violations were noted at time of inspection.

Met with Inflection representatives Ross Drjnevitch and Greg Malekos on location. Rig was currently drilling the 22" hole for the 18 5/8" surface casing. Rig was estimating to TD the surface casing hole at approximately 764'. They then intend to hang the casing and skid the rig to drill the surface casing holes for the 5H and the 11H. They will then skid back to the 10H and begin drilling the 17 1/2" hole for their intermediate casing. No violations were noted at time of inspection.
