| At the time of inspection, well was producing.  Well integrity appeared to be in good condition, with no signs of any leaks.  There was some minor rust.  The cellar was filled with gravel.  The pressure reading for the tubing was reading 295 psi and casing was 450 psi.  The well was being injected with a foamer combo.  The well was properly labeled and the Emergency Response sign was posted at the site entrance. There were 4 tanks on the pad.  3 were contained within a lined, steel dike and 1 was not contained.  3 line heaters were in use, 1 being a dual heater.

The well was properly labeled and the Emergency Response sign was posted at the site entrance. There were 4 tanks on the pad. 3 were contained within a lined, steel dike and 1 was not contained. 3 line heaters were in use, 1 being a dual heater.
