| The Department inspected the Talisman Energy USA, Inc. Storch well pad (ESCGP#ESX09-015-0086) on 7/20/2015 @ 1400 hours. An adequate site ID sign was observed at the entrance to the access road. The company E&S inspection log indicated the site was last inspected on 7/1/2015. The receiving stream is Beckwith Creek (CWF) and the ESCGP covers an 11.7 acres disturbance.

| The DEP conducted a RESTORATION inspection of Repsol Oil & Gas's Storch D (03-035) well site in Wells Twp, Bradford County on 10/22/2018 at 1125 hours. The weather was cloudy, 39 degrees and 64% humidity.

The inspection was conducted in response to a Notice of Termination received by the DEP. The site currently has one producing well and seven plugged wells. The site is well vegetated, stabilized and no temporary E&S BMPs were observed. All PCSM practices are in place and appear to be functioning properly. A well site restoration report was accepted by the DEP.

I recommend this Notice of Termination was approval. No violations noted at this time. I departed the site at 1150.

The company E&S inspection log indicated the site was last inspected on 7/1/2015. The receiving stream is Beckwith Creek (CWF) and the ESCGP covers an 11.7 acres disturbance.

2792640 Primary Facility 2018-10-22 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of Repsol Oil & Gas's Storch D (03-035) well site in Wells Twp, Bradford County on 10/22/2018 at 1125 hours. The weather was cloudy, 39 degrees and 64% humidity.

The inspection was conducted in response to a Notice of Termination received by the DEP. The site currently has one producing well and seven plugged wells. The site is well vegetated, stabilized and no temporary E&S BMPs were observed. All PCSM practices are in place and appear to be functioning properly. A well site restoration report was accepted by the DEP.

I recommend this Notice of Termination was approval. No violations noted at this time. I departed the site at 1150.
