The well is equipped with a large Texas Style gas plunger lift system with a steel cage built over the well. The production master valves are open, and the annular valves are open measuring pressure. The tubing pressure is 0 psi and the annulus are reading 0 psi.
0042217321 2018-11-08 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-4336. At the time of my inspection the well was temporarily abandoned and caged while Patterson #343 was rigging up to drill two additional wells on the pad. Jersey barriers were placed outside of the cage to prevent a collision with the well head. Pressure gauges to monitor annular, production, and tubing pressure remained in place. The intermediate casing annular pressure gauge indicated 100 psi. The production casing annulus indicated 10 psi. The casing and tubing gauges both indicated 0 psi. All valves remained in the closed position. No leaks were observed. The flare stack was positioned in the Southwest corner of the pad and away from the well heads and surface equipment. The proposed and approved SIMOPS plan conditions were met at this time.

The well was recently turned back on after being temporarily abandon in order for two addition wells to be drilled on pad. During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of sixteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and two combustors.
0293903673 2018-09-25 Production Wells No Violations Accompanied by ODNR Kenn Frye, during our inspection a berm was being constructed around the pad with the cuttings from the conductors. Conductors had been cemented for 3405924532 and 3405924533. The well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of fifteen 500-barrel tanks, four gas production units, two gas compression skids, four vapor recovery towers, horizontal vent scrubber, and one combustor. Kevin Rothenbuhler was contacted about the cellars bubbling on the 3H and 4H. Mr. Rothenbuhler stated that they would remove the pea gravel from the cellars on all wells and tighten valves and fittings that are plumbed up for annular pressure monitoring to remediate the bubbling.

1467999759 2016-07-08 Production Wells No Violations I received a call on 7/7/16 at 22:00 from PDC EHS Supervisor Tony Long stating that at 18:45 an Ergon truck had a 2bbl oil release on the pad and that Energy Services had been dispatched to clean-up the release. Energy Services had put contained release, applied absorbent pads and bio sorb to the affected area but due to light shut-down final remediation. Energy Services was dispatched again on 7/8/16 at 09:30 and removed stone, remediated with clean fill. Upon arrival at 12:00 remediation was just being completed and contaminated stone will be hauled to a solids landfill.

Identification was present and production records were current. Utica Operating Resources has been notified to identify the gauges for annular pressure monitoring.

QES was on location with a nitrogen truck injecting nitrogen into the 4H well for Enhanced Recovery.

Kevin Rothenbuhler contacted me and stated that the pea gravel was removed from the cellar, the valves for annular pressure monitoring were tightened, and while utilizing a gas sniffer no leaks were detected. The pea gravel was placed back in the cellars today and no bubbling was present.

The site passed preliminary restoration according to ORC 1509.072A.
1873380787 2018-09-25 Production Wells No Violations The purpose of my inspection was to document the status of Utica Resource Operatings Dynamite Pad prior to SIMOPS activities. Guerney County Inspector Austin Gutridge was present during the inspection. At the time of inspection well #2-4336 remained in production. Correct identification was present. A total of four (4) production wells were on the pad. Well #2-4336 will be within the fall radius of the anticipated rig contracted to drill the two permitted wells on the West side of the pad. Approved temporary abandonment permit conditions will need to be completed on well #2-4336 prior to the commencement of drilling activities that include well #2-4336 within the fall radius of the derrick.
