2208632 Primary Facility 2013-09-29 Incident- Response to Accident or Event Violation(s) Noted On 9/27 13 at approximately 9:45 AM SWN contacted the Department to report the discovery of a small production fluid spill, 5-10 gallons, which was identified at approximately 8:45 AM. The spill is on the SWN RU 10 Eastman Pad and was caused by a leak which developed in a production unit. SWN contacted the Department at 11:45 AM to report that the spill appears to be bigger than had been originally reported that estimates now place the volume in the vicinity of 100 plus gallons. SWN is in the process of shutting in the well and then will remove the production equipment to facilitate the ensuing remediation effort. Today, 9/30/13, Jon Sassi and I arrived on site and met Seth Tratthen, SWN Regulatory Analyst.
2331966 Primary Facility 2014-01-02 Administrative/File Review No Violations Noted On September 27, 2013, the Oil & Gas Program was notified that a leaking production unit released 100+ gallons of brine to the soil. ECB received a remediation report for the incident on October 18, 2013. Since the report was submitted under the “alternative remediation” process, as described in the “Addressing Spills and Releases at Oil & Gas Well Sites” policy, ECB provided the Oil & Gas Program with a memo dated October 23, 2013 stating that the attainment sampling was deficient. ECB received another remediation report for the incident on December 18, 2013. On December 31, 2013, ECB provided the Oil & Gas Program with a memo stating that the impacted area was remediated to background standards (except Mg). Act 2 release of liability cannot be granted for this incident since the report was submitted under the “alternative remediation” process, as described in the policy “Addressing Spills and Releases at Oil & Gas Well Sites.” e-Mails forwarded to SWN 1/2&3/2014 closing this matter.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed.
