Spoke to DSM Ken Silvis who agreed to place the affected gravel on containment and agreed to remove the snow pile.Noted erosion issue from previous inspection on 11/28/12 (ID 2121600) was addressed by utilizing an erosion blanket for stabilization and removing sediment from ditch.Rig and on-site tanks on containment. Tanks and containment appear structurally sound. Access road adequately rocked and no accelerated E&S issues were noted.Department recommends that Operator remove debris from rock-lined ditches.

Site ID sign, valid permit, E&S plan, ESCGPI, PPC Plan present at access road entrance. Security on-site. At the time of this inspection, cellar for 4H constructed. Actively drilling 5H, 1H had been drilled, cellars for 2H,3H constructed, 6H staked. Minor erosion on hillside observed on back left corner of pad when facing wellheads from access road. Minor sediment from this erosion observed at base of hill in rock-lined ditch. Observed tophole cuttings along soil pile. Department spoke with company representative who stated plans were to incorporate cuttings into soil pile and disperse across pad. Operator will address erosion concerns by routinely monitoring this area in order to prevent pollution event due to accelerated erosion and sedimentation. Access road adequately rocked and no E&S issues were noted in relation to access road. Department recommends that Operator continues to monitor and maintain access road and well-site as necessary.

Keane 2144 rig shanked a bit and decided to stop drilling hole. Chevron, Natasha Sackivichik (Drilling Engineer) called DEP and informed DEP of skidding rig to well 6H.. Chevron plugged well 4H will 910 sacks of Class A cement.

2144637 2013-02-25 No Violations Noted Multi-well pad. Site ID sign, valid permit, E&S plan, ESCGPI, PPC plan all present.. Security on-site.At time of inspection, in process of skidding rig to resume drilling. 4H spudded, TD not yet reached. 1H had been drilled. 2H, 3H, 5H, 6H also all spudded, TDs not yet reached.Observed small staining on gravel on front edge of rig containment when standing at DSM trailer and noted large snow/gravel pile against brine tanks in back corner of pad which could lead to a potential pollution event by falling into ditches after thawing. Spoke to DSM Ken Silvis who agreed to place the affected gravel on containment and agreed to remove the snow pile.Noted erosion issue from previous inspection on 11/28/12 (ID 2121600) was addressed by utilizing an erosion blanket for stabilization and removing sediment from ditch.Rig and on-site tanks on containment. Tanks and containment appear structurally sound. Access road adequately rocked and no accelerated E&S issues were noted.Department recommends that Operator remove debris from rock-lined ditches.

DEPARTMENT FOLLOWED UP WITH OPERATOR AND IT APPEARS THAT OPERATOR IS IMPLEMENTING AND FOLLOWING CURRENT SPILL POLICY AND NOTIFIED THE DEPT AND BEGAN CLEAN UP MEASURES IN A TIMELY MANNER.

Production pipe was set and was cemented back to surface with 1730 sacks of Varicam (lead) 400 sks SoloCam (tail) with 32 bbl return. Haliburton performed the cementing of the production pipe.

2235871 2014-01-13 No Violations Noted On 1/13/14 inspected this 6 well, well pad. The weather the day of the inspection was unusually warm and wet.

The entrance road to the well pad was well graveled. No mud was being drug out onto the main road.

Checked in with the guard.

Walked over the impoundment and met with Harvey Horner with Nuverra. Nuverra was working on the small truck turn around pad that is adjacent and on the Northern side of the impoundment. They were adding stone to the pad.

Walked over to the well pad. The well pad was inspected.

The # 1 well was not marked. Please mark this well and notify this inspector when the well is marked.

Large plastic pipe was being unloaded onto the well pad.

No signs of spills were noticed on the well pad.

2244509 2014-02-10 No Violations Noted Met with Curtis Small and inspected the well pad. Chevron was setting up on the well pad to frack a number of wells. The frack tanks will block access to the the pipeline companies access to their pipeline controls. Chevron propsed adding an additional ramp tp allow the pipeline company access to their well controls. The access road will only be temporary.
2250981 2014-03-05 No Violations Noted On 3/5/14 met with Joe Brusco with Chevron and inspected the site.

The sign was correctly marked.

No mud was being drug out to the main road.

Checked in with the guard and donned the required PPE.

They were preparing the well pad for the frack equipment that will be coming on site. A road leading up to the Caymen pipeline was being reworked. This will allow the Caymen workers will have access to the pipeline.

A minor gas leak was observed at the Caymen pipeline. Chevron called Caymen and a Caymen worker responded to the site and repaired the leak.

The equipment on site was checked for leaks. No leaks were observed. No staining on the lined well pad was observed.

2252447 2014-03-10 No Violations Noted On 3/10/14 met with Alex Grant with Chevron and inspected the site.

2311283 2014-10-08 No Violations Noted Work was underway to equip a well for production. A coil tubing rig was located onsite. Secondary containment was located on the well pad and there were 10 green and 4 red trailer tanks located onsite on top of the containment. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that above and on either side of the site there was a cultivated crop field with soybeans.
