| FOLLOW UP TO NOV #  1282048725 On day of inspection I was in on site for a BOP Test. The BOP test could not be completed at present time because of wrong rubber size for the job. The CNX engineer Jay Hewitt notified me on a situation with water leaking out of hill. There were four places. There was a 4''flow and two 2''flows.There was also a 8''plastic corrugated pipe that was used for a french drain between the pit and the west wall of the location. I contacted Supervisor Simmers and Inspector Moravy. They told me to notify Inspector Farnsworth to assist me with this situation. I contacted Inspector Farnsworth and he would be in route ASAP.I started taking samples of the water.WS-1 was taken at the 4''stream down over the north end of the pit.WS-2 was taken at the sediment trap.WS-3 was taken at the center of the pit.Quantab readings revealed that the chlorides were 10,000-13,000ppm at WS-1 and WS-2.WS-3 and at site behind rig indicated that the chlorides were higher than that.Baroid employee Titus Robinson took samples also. He indicated that the ground water sample was at a chloride level at 13,000ppm.The pit sample was at a 28,000ppm in chlorides. Jay Hewitt contacted the construction team and advised them that he needed them on site to construct the containment pit down over hill. At this point in time, the source of water is yet to be determined. I contacted Tom Tomastik in Columbus and he advised that there were no wells in area near the CNX site.Delrick construction showed up and started to get prepared to construct the containment pit. Approximately 6:00pm Inspector Farnsworth arrived and Hewitt and I briefed him about the situation. We assessed the pit and the water flowing out of the hillside. There was no visible indication on the pit liner that there was a fluid loss from the drilling pit at this time. Inspection of the drilling pit revealed that pit liner was scorched and melted during the flaring of the Utica gas. We observed that the fluid level was at the lowest visible hole at this time. We tied an empty plastic sample bottle onto the flare line so that it was touching the fluid level so we could use this as fluid level indicator. Later in the evening we observed the water level was approximately one inch below the plastic bottle and the was a visible ring on the pit liner indicating a fluid loss. The drilling pit was losing fluid earlier in the evening , but due to the hot sun evaporating the fluid on the liner it was difficult to see the fluid loss. We notified Jay Hewitt that there was a visible fluid loss at this time and he immediately contacted water trucks and he also advised the drilling rig crew to start pumping the drilling pit into frac tanks. During the course of removing fluid from the drilling pit , we noticed a significant loss of water flow on the hillside. We continued to monitor the removal of the fluid from the drilling pit and the water flow on the hillside thru out the night and into the early morning. We advised Jay Hewitt to pump the fluids from the sediment trap and to keep pumping the fluid from the drilling pit until the water quits flowing out of the hillside. He advised that his company would empty the drilling pit and reline the drilling pit. We advised that they could pump it down below the holes or the could empty the pit and reline it. The operator is taking the appropriate remedial actions at this time. I advised Jay Hewitt that I would return to the site later today (Friday 13th) to check on compliance.

| On day of inspection I was in on site for a BOP Test. I contacted Inspector Farnsworth and he would be in route ASAP.I started taking samples of the water.WS-1 was taken at the 4''stream down over the north end of the pit.WS-2 was taken at the sediment trap.WS-3 was taken at the center of the pit. Quantab readings revealed that the chlorides were 10,000-13,000ppm at WS-1 and WS-2.WS-3 and at site behind rig indicated that the chlorides were higher than that. The Baroid employee Titus Robinson took samples also. I contacted Tom Tomastik in Columbus and he advised that there were no wells in area near the CNX site. Delrick construction showed up and started to get prepared to construct the containment pit. We notified Jay Hewitt that there was a visible fluid loss at this time and he immediately contacted water trucks and he also advised the drilling rig crew to start pumping the drilling pit into flow back tanks.

| On day of inspection it was raining I had met with Jim Black on the site. It was causing soap suds on hillside and I told Black to get some environmental friendly de foamer sent to site. I was told that a tote of biodegradable soap was opened up and causing the problem. Up on inspection found that there was 25 roll offs on site and the company was loading them with pit cuttings. They will remove the pit liner and install fire clay in the bottom of the pit. All cuttings are hauled to an approved landfill.

| On day of inspection I checked the drill pit and the area on hillside where water was coming out. Water was at a small tricle. Water trucks were hauling out of the pit. I took Quantab samples from the holes on the hill down to where there is a lined containment pit and down to culvert at St.Rt.800.The chlorides were at 5800 ppm. From the culvert to creek that runs parallel with State Route 800 The Chlorides were at .01 Parts Per Million. I told Chris Miller the CNX Consultant to haul out of the pit and the trap. Line the trap and from now on haul the fluids. I will be stopping in to Quantab the site and check chlorides.

1104872061 2010-12-09 On day of inspection drill pit has been covered. Pit liner and French drain has been removed. There is no more seepage out of the bank. Compliance Notice has been resolved.
-1133218044 2014-03-20 Team Service rig on site to release packer and pull tubing, it was noted that packer has been released and they were placing 10# brine in well so it would not flow during packer removal.
-1162343719 2013-01-16 Inspection revealed that operator was waiting on enough water to be received so he could start injection pump it was noted that Force Well Service had one vacuum truck unloading at facility during my inspection.

Note: Operator stated that well was taking approximately 1400 bbl of brine per day.

-1178263128 2014-08-19 Inspection revealed that injection pump was in operation during my inspection and it was noted that the cage for the wellhead has been put back in place.
-1182533367 2011-03-28 This well has been drilled and is currently shut in and awaiting completion work. There is 10,000 PSI frac valve on the well head. The earthing pits were found to be in good condition.
1211769218 2012-02-16 Inspection revealed that 8-300 bbl tanks and 2-400 bbl tanks and 1-100 bbl tank were set within dike area and pump house and electrical building is under construction will monitor site too make sure contractor follows site plans established for said well.
-1236205293 2010-08-12 FOLLOW UP TO NOV # 1282048725 On day of inspection I was in on site for a BOP Test. The BOP test could not be completed at present time because of wrong rubber size for the job. The CNX engineer Jay Hewitt notified me on a situation with water leaking out of hill. There were four places. There was a 4flow and two 2flows.There was also a 8plastic corrugated pipe that was used for a french drain between the pit and the west wall of the location. I contacted Supervisor Simmers and Inspector Moravy. They told me to notify Inspector Farnsworth to assist me with this situation. I contacted Inspector Farnsworth and he would be in route ASAP.I started taking samples of the water.WS-1 was taken at the 4stream down over the north end of the pit.WS-2 was taken at the sediment trap.WS-3 was taken at the center of the pit.Quantab readings revealed that the chlorides were 10,000-13,000ppm at WS-1 and WS-2.WS-3 and at site behind rig indicated that the chlorides were higher than that.Baroid employee Titus Robinson took samples also.
-12674869 2010-08-02 On day of inspection well was showing large flares at 8464'and at 8474'.They were at 1575 units on air.Haliburton Engineer is showing up. They will go to fluid. They plan to mix up 9.1weight and not go over 9.3ppg.
-1275814882 2014-03-06 Fishing for lost tools within tubing string.
1280930825 2010-08-04 On day of inspection Les Wilson rig running in with pipe and overshot and jars to retrieve the fish. The Total depth is at is at 8474'.At 8425 is where the bit is at.1200 bbl was what it should have took to load hole.1500bbl is what was actual. Lost circulation and started pulling drillpipe. Will trip out all pipe out of hole and mud up.
1281878267 2010-08-15 At about 9:00am Chris Miller the drilling consultant with CNX called and stated that the water quit running out of hill. Water trucks are not available today but water is contained. Chris stated that he has pictures. The trap/impoundment has been pulled down and they will line with a pit liner. I stated that we will continually monitor the chlorides.

We notified Jay Hewitt that there was a visible fluid loss at this time and he immediately contacted water trucks and he also advised the drilling rig crew to start pumping the drilling pit into flow back tanks.
