| When I arrived at the tank battery, the injection pump was running.  I immediately called Danny Baker and told him that an injection line test needed to be administered, and witnessed by the state before he could run the pump.  This test was supposed to have taken place in the summer of 2011I was then joined by Jon Scott south region UIC inspector. When Danny Baker arrived with his crew, two attempts were made to test the flow line.  Both had failed.  Inspector Kip Thompson was also present on this inspection.

| An inspection of the SWIW and related storage tanks was conducted with Mr. Jeff Fry.  During the inspection, an old bare area with exposed flowline were observed below the storage tanks located in a low lying area near the creek.  Note: Timber has been removed from this locaton, but signs of stressed vegetation were noted.  It was also noted that general housekeeping would be needed at the unloading pad/concrete vault location.  Old oil found around the concrete vault and appeared to be breaking down.  Old filters along with a drum containing oily debris, which may have leaked would need to be placed inside dike to avoid leakage.  At the time of the inspection, the meter showed 224,481 bbls.  Left voice mail message for the pumper, Mr. Danny Baker with Jeanie Enterprises on 6/2/2009 at 2:12 pm, in regards to the inspection.  Mr. Baker returned call on 6/3/2009 and confirmed line break over the hill last year.  Mr. Baker was instructed to place lime, seed and mulch on the impacted area.  Perform general housekeeping and recommended filters and drum placed inside dike.

| 1-16-12 Received call from Andy Thomas MRI, that a brine tank had flowed over and into the dike. 1-17-12 made arrangements to meet at tank battery. The tanks had been recently wiped down and the dike had been cleaned out. The spill was contained with in the dike. The injection line from the pump to the injection well has been worked on recently and I requested that they test the line to show mechanical integrity. Two attempts were made to test the injection line. Both attempts were unsuccessful. The pumper requested that they shut down the injection and make repairs to the line on the 1-18-12 then re-test on 1-19-12.

GPS coordinates N 39.56988, W 82.43575.  Due to lack of vegetation, it was suspected that there had been a brine spill.  Quantab results from a puddle of water in the vegetation kill area showed that the chloride content was too high for the Quantab to quantify.  Mr. Danny Baker was notified by a phone call, and instructed to remediate soil where the vegetation was killed and build a containment dike around the pump because Mr. Baker had suspected it was leaking.  He was also advised of other potential plumbing risks associated with the pump.  Mr. Baker stated that he would be out there the following day with a piece of equipment to remediate situation.

| When I arrived at the tank battery, the injection pump was running.  I immediately called Danny Baker and told him that an injection line test needed to be administered, and witnessed by the state before he could run the pump.  This test was supposed to have taken place in the summer of 2011.  I was then joined by Jon Scott south region UIC inspector.  When Danny Baker arrived with his crew, two attempts were made to test the flow line.  Both had failed.  Inspector Kip Thompson was also present on this inspection.

| An inspection of the SWIW and related storage tanks was conducted with Mr. Jeff Fry.  During the inspection, an old bare area with exposed flowline were observed below the storage tanks located in a low lying area near the creek.  Note: Timber has been removed from this locaton, but signs of stressed vegetation were noted.  It was also noted that general housekeeping would be needed at the unloading pad/concrete vault location.  Old oil found around the concrete vault and appeared to be breaking down.  Old filters along with a drum containing oily debris, which may have leaked would need to be placed inside dike to avoid leakage.  At the time of the inspection, the meter showed 224,481 bbls.  Left voice mail message for the pumper, Mr. Danny Baker with Jeanie Enterprises on 6/2/2009 at 2:12 pm, in regards to the inspection.  Mr. Baker returned call on 6/3/2009 and confirmed line break over the hill last year.  Mr. Baker was instructed to place lime, seed and mulch on the impacted area.  Perform general housekeeping and recommended filters and drum placed inside dike.

| A follow-up inspection regarding the leak from a saltwater drop tank (Reed Road) revealed non-compliance.  The fluid inside the trench showed 3.8 units on Quantabs (High Range), but was not discharging into the creek.  More fluid will need to be pumped from this trench.  An elbow fitting, on a steel flow line, that crosses the creek was observed to contain a small leak.  Quantab showed maximum of 10 units.  The company was contacted and informed of the inspection results.

| 1-16-12 Received call from Andy Thomas MRI, that a brine tank had flowed over and into the dike. 1-17-12 made arrangements to meet at tank battery. The tanks had been recently wiped down and the dike had been cleaned out. The spill was contained with in the dike. The injection line from the pump to the injection well has been worked on recently and I requested that they test the line to show mechanical integrity. Two attempts were made to test the injection line. Both attempts were unsuccessful. The pumper requested that they shut down the injection and make repairs to the line on the 1-18-12 then re-test on 1-19-12.

1154697983 2006-07-25 An inspection was conducted at the facility owned by Mr. Mark Schmelzer on 7/25/2006. During the inspection, it was noted that the injection line had a small pinhole leak in the injection line. As a result, brine was being discharged directly onto the ground surface. Also noted was that the surrounding area contained dead vegetation. Mrs. Gene Schmelzer was immediately contacted by telephone @11:15am on 7/25/2006. She was informed to correct this problem immediately. In regards to the SWIW inspection, it was found that a ID will be needed at the well. The ID the storage tanks contains the wrong area code. The dike needs to be pumped and the oily soil and material needs to be removed from the lease. On 7/28/2006, a follow-up inspection was conducted to ensure that the leaking fitting has been replaced. During the inspection, it was noted that the fitting was replaced and straw had been applied to the surrounding area.

Due to lack of vegetation, it was suspected that there had been a brine spill. Quantab results from a puddle of water in the vegetation kill area showed that the chloride content was too high for the Quantab to quantify. Mr. Danny Baker was notified by a phone call, and instructed to remediate soil where the vegetation was killed and build a containment dike around the pump because Mr. Baker had suspected it was leaking. He was also advised of other potential plumbing risks associated with the pump. Mr. Baker stated that he would be out there the following day with a piece of equipment to remediate situation.

And the fluid inside the leaking tank will be transferred over to the injection well. The pumper states that they found this leak on the 3/5/2008. A new 100 bbl tank was observed on-site. Dennis Deavers with OEPA was informed of the inspection results.

Mr. Baker states that they have been working on cleanup and have taken oily soils to the landfill and will provide run tickets. He was informed that the impoundment will also need to be addressed along with the areas outside the dike.

When Danny Baker arrived with his crew, two attempts were made to test the flow line. Both had failed. Inspector Kip Thompson was also present on this inspection.

705372691 2013-06-26 I conducted a inspection of the lift pump station. This pump lifts brine to flow to their injection well. The fittings on the pump are leaking, the brine is being collected in a pit. The pit is not liquid tight allowing brine to leach thru. I sampled the brine using high range chloride titrators. I sampled two areas both samples registered 2400 PPM chlorides. Mark Smeltzer shut the pump off at 1322 hours. I asked the pump not be used until the leak is corrected and the dike is made liquid tight. The pumping station is located on reed road.

-818713486 2009-06-02 An inspection of the SWIW and related storage tanks was conducted with Mr. Jeff Fry. During the inspection, an old bare area with exposed flowline were observed below the storage tanks located in a low lying area near the creek. Note: Timber has been removed from this locaton, but signs of stressed vegetation were noted. It was also noted that general housekeeping would be needed at the unloading pad/concrete vault location. Old oil found around the concrete vault and appeared to be breaking down. Old filters along with a drum containing oily debris, which may have leaked would need to be placed inside dike to avoid leakage. At the time of the inspection, the meter showed 224,481 bbls. Left voice mail message for the pumper, Mr. Danny Baker with Jeanie Enterprises on 6/2/2009 at 2:12 pm, in regards to the inspection. Mr. Baker returned call on 6/3/2009 and confirmed line break over the hill last year. Mr. Baker was instructed to place lime, seed and mulch on the impacted area. Perform general housekeeping and recommended filters and drum placed inside dike.

849615643 2009-10-01 An inspection of the saltwater drop or pump station to Roberston (SWIW #1) was conducted. The facility contains only one (1) 100 bbl. The second 100 bbl that contained a hole and was leaking has now been removed. Currently, the existing 100 bbl contains a leak (approx 1 drip every 10 sec) on bottom at valve with swage before connection to PVC elbow fitting. A rag has been placed around the leak. High Range Quantabs exceeded 10 units of Cl-. Note: 8.4 units equals 6545 ppm (mg/L) of Cl-. The attached line is 2 Sch 40 PVC pipe routed to pump house (inlet). A leak was also observed at this location (inlet to pump). 1 drip approx every 60 seconds. The outlet is 2 steel from pump house uphill to SWIW. Old oil observed a pump house with wood chips placed around location. The oil came derives from a rotted steel line that had been repaired. Wash from rainfall events drains from location (pump house and tank) to creek approx 30' and 10' away respectively. The earthen dike around 100bbl tank contains a trench. The pumper, Mr. Danny Baker arrived at location during inspection. He was instructed of deficiencies and made attempt to stop leaks. Mr. Baker states cleanup, dike and water diversion will be done next week.

-858432577 2008-03-13 A follow-up inspection regarding the leak from a saltwater drop tank (Reed Road) revealed non-compliance. The fluid inside the trench showed 3.8 units on Quantabs (High Range), but was not discharging into the creek. More fluid will need to be pumped from this trench. An elbow fitting, on a steel flow line, that crosses the creek was observed to contain a small leak. Quantab showed maximum of 10 units. The company was contacted and informed of the inspection results.

-994351388 2012-01-17 1-16-12 Received call from Andy Thomas MRI, that a brine tank had flowed over and into the dike. 1-17-12 made arrangements to meet at tank battery. The tanks had been recently wiped down and the dike had been cleaned out. The spill was contained with in the dike. The injection line from the pump to the injection well has been worked on recently and I requested that they test the line to show mechanical integrity. Two attempts were made to test the injection line. Both attempts were unsuccessful. The pumper requested that they shut down the injection and make repairs to the line on the 1-18-12 then re-test on 1-19-12.
