| I inspected this well as part of routine inspection activity. At the time of this inspection, I found that a large tree had fallen onto a portion of the tank battery damaging the horizontal separator unit. The well had been shut-in and no apparent spillage or leaks had resulted. I contacted the well owner who had previously discovered the damage and was in the process of making repairs.

| I inspected this well as part of routine inspection activity. At the time of this inspection, fittings and connections were secure and free from leakage or problems. The tank lid was closed. The load line was plugged and secure. Production reports were current. Bond and insurance for this operator were current. There were 2 to 3 inches of fresh snow around the area. I walked and inspected the production elements as well as the wooded portions around the well and battery and found no indications of problems. The tree fall that had occurred prior to my last inspection had been removed and the damage to the system elements repaired.

| I inspected this well location as part of routine inspection activity. At the time of ths inspection, I observed th following: a wellhad with pump jack system and associatd tank battery located in a small clearing in an otherwise wooded area. The tank batery was comprised of one 210-barrel steel production tank and separator unit. The load line valves were closed and secure, but were not locked.  The tank lid was closed and sealed. Fittings and connections were secure and free from signs of leakage. There was secondary containment and it was intact and free from signs of leakage or spills. I noted that there was an indirect fire heater unit configured within the tank battery. I notifed Paul Mihalik that the indirect heater must be at least 50 feet from the well and from the storage tank, so it may not be operated in its current configuration.

| I inspected this well as part of routine inspection activity. My inspection found the following: Access to the well is via a dedicated access road located just east of the Red Mill Golf facility. At the time of this inspection, there are 14+ inches of snow cover in the area. As such, portions of the producing system are under snow cover. | I inspected this well as part of routine inspection activity. My inspection found the following: Access to the well is via a dedicated access road located just east of the Red Mill Golf facility. At the time of this inspection, there are 14+ inches of snow cover in the area. As such, portions of the producing system are under snow cover. The access road has not been plowed, access to the wellhead and battery are accessible by foot only. Connections at the wellhead pump jack and tank battery are secure with no signs of leakage. At the time of this inspection, I found that a large tree had fallen onto a portion of the tank battery damaging the horizontal separator unit. The well had been shut-in and no apparent spillage or leaks had resulted. I contacted the well owner who had previously discovered the damage and was in the process of making repairs.

At the time of this inspection, fittings and connections were secure and free from leakage or problems. The tank lid was closed. The load line was plugged and secure. Production reports were current. Bond and insurance for this operator were current. There were 2 to 3 inches of fresh snow around the area. I walked and inspected the production elements as well as the wooded portions around the well and battery and found no indications of problems. The tree fall that had occurred prior to my last inspection had been removed and the damage to the system elements repaired.

At the time of ths inspection, I observed th following: a wellhad with pump jack system and associatd tank battery located in a small clearing in an otherwise wooded area. The tank batery was comprised of one 210-barrel steel production tank and separator unit. The load line valves were closed and secure, but were not locked. The tank lid was closed and sealed. Fittings and connections were secure and free from signs of leakage. There was secondary containment and it was intact and free from signs of leakage or spills. I noted that there was an indirect fire heater unit configured within the tank battery. I notifed Paul Mihalik that the indirect heater must be at least 50 feet from the well and from the storage tank, so it may not be operated in its current configuration.
