At the time of this inspection I observed the following: a wellhead with plunger-lift system and associated battery on a larger parcel occupied by a church and large, asphalt-paved parking area. The individual enclosure gates were closed, locked, and secure. Load line valves were locked and secure. Secondary containment was intact. Areas surrounding the well and battery were comprised of asphalt-paved parking and drive areas. I walked and inspected the surrounding areas and found no signs of impacts.

Ingress and egress to and from the well and battery is afforded by the asphalt-paved parking and drive which is utilized and maintained by the church. The gates and enclosure at the battery are closed, locked, and secure. Load-outs are locked, bull plugged, and secure. Tank hatch lids are closed, sealed, and secure. Process safety equipment (spark arrestor, low-pressure relief valve, and carbon filter) is present. The interior of the secondary containment is dry and free of signs of leakage or spills. The battery enclosure requires two gates . The ID signage does not contain owner/operator address and contact information as required. I have advised Duck Creek Energy regarding these deficiencies.

| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: A wellhead with plunger lift system in a board-on-board enclosure. The enclosure gate was locked and secure, Fittings and connections at the wellhead were secure. The 8.625-inch casing was plumbed to surface and open to atmosphere with no gas emission noted. The well and associated enclosure is located in an asphalt-paved parking area otherwise utilized by a church for parking. There was no evidence in and around the well enclosure of leakage or problems. The associated tank battery is located on the north side of the church facility, and similarly located in an asphalt-paved parking area. The battery elements are secured within a board-on-board enclosure. The entry gate was locked and secure. The load-line valves were plugged, locked, and secure. The tank lids were closed and secure. I inspected the interior areas of the battery and found the fittings and connections secure. The secondary containment was intact and dry with no evidence of spills or leakage.

At the time of this inspection I observed the following: a wellhead with plunger-lift system and associated battery on a larger parcel occupied by a church and large, asphalt-paved parking area. The individual enclosure gates were closed, locked, and secure. Load line valves were locked and secure. Secondary containment was intact. Areas surrounding the well and battery were comprised of asphalt-paved parking and drive areas. I walked and inspected the surrounding areas and found no signs of impacts.

The pumpjack and tank battery are enclosed in separate board-on-board enclosures which are locked. The tank battery consists of 2 140-barrel tanks and mechanical separator. Also enclosed within the battery is a Dominion metering run. Safety equipment is present in accordance with 1501:9-9-05E. Secondary containment is present. Production records are current. The gate to the tank battery area is locked and does not have any ID placard. Otherise, no problems or violations. Preliminary restoration passes.

Process safety equipment (spark arrestor, low-pressure relief valve, and carbon filter) is present. The interior of the secondary containment is dry and free of signs of leakage or spills. The battery enclosure requires two gates . The ID signage does not contain owner/operator address and contact information as required. I have advised Duck Creek Energy regarding these deficiencies.

The enclosure gate was locked and secure, Fittings and connections at the wellhead were secure. The 8.625-inch casing was plumbed to surface and open to atmosphere with no gas emission noted. The well and associated enclosure is located in an asphalt-paved parking area otherwise utilized by a church for parking. There was no evidence in and around the well enclosure of leakage or problems. The associated tank battery is located on the north side of the church facility, and similarly located in an asphalt-paved parking area. The battery elements are secured within a board-on-board enclosure. The entry gate was locked and secure. The load-line valves were plugged, locked, and secure. The tank lids were closed and secure. I inspected the interior areas of the battery and found the fittings and connections secure. The secondary containment was intact and dry with no evidence of spills or leakage.
