| At the time of this inspection, the well was equipped with a plunger lift assembly.  The annulus and flow line valves were open.  A domestic gas supply was plumbed off of the wellhead with an open ball valve, a regulator, 3 inch drip tank and gas meter.  There was identification posted at the well.  No fencing was installed around the well, due to a pre-site waiver.  The diked tank battery has two storage tanks and a separator.  The 50 BBL poly tank had a leak at the hammer union on the dump line.  The leak needs repaired.  The pick-up lines have plugs and locks.  The 210 BBL tank was missing the required urban safety equipment and needs installed per 1501:9-9--e).  There was an identification sign posted at the tank battery.  The identification sign was missing the required information per 1501:9-9-05(E)(1)(a-e).  A locked chain link fence surrounds the tank battery.  The 210 BBL tank extends more than 3 foot above the fencing.  Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9--site inspection.  Install the required gate with a lock at the access road entrance per 1501:9-9--9-

| I conducted a compliance notice follow-up inspection for compliance notice 152402007, requiring multiple compliance issues involving urban requirments.  At the time of this inspection, the  hammer union on the dump line of the 50 BBL poly tank has been repaired.  The 210 BBL tank was still missing the required urban safety equipment and needs installed per 1501:9-9-05E(4)(b-e).  There was an identification sign posted at the tank battery that has been amended with the required information per 1501:9-9-05(E)(1)(a-e).  A locked chain link fence surrounds the tank battery.  The 210 BBL tank extends more than 3 foot above the fencing.  Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f).  There was no landscaping around the tank battery, however the tank battery was located in a wooded area.  No gate has been installed at the entrance.  The gate was not waived during the pre-site inspection.  Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2), and post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f).

| I was on location for a status check.  During my inspection, I found the well in production.  The well is equipped with plunger lift assembly.  The tank battery consists of a 210 barrel production tank, a 50 barrel, poly brine tank, and a vertical, mechanical separator.  There is dike around the tank battery.  An identification sign is posted.  There is no urban safety equipment on the production tank.  There are locked, chain link fences surrounding the well and tank battery.  The lease road is in stable condition.  I called and spoke with PAC Drilling.  I instructed PAC to immediately install the proper urban safety equipment on the production tank.  Follow-up inspections will be conducted.

A domestic gas supply was plumbed off of the wellhead with an open ball valve, a regulator, 3 inch drip tank and gas meter. There was identification posted at the well. No fencing was installed around the well, due to a pre-site waiver. The diked tank battery has two storage tanks and a separator. The 50 BBL poly tank had a leak at the hammer union on the dump line. The leak needs repaired. The pick-up lines have plugs and locks. The 210 BBL tank was missing the required urban safety equipment and needs installed per 1501:9-9--e). There was an identification sign posted at the tank battery. The identification sign was missing the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence surrounds the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9--site inspection. Install the required gate with a lock at the access road entrance per 1501:9-9--9-
0186385100 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 152402007, requiring multiple compliance issues involving urban requirments. At the time of this inspection, the hammer union on the dump line of the 50 BBL poly tank has been repaired. The 210 BBL tank was still missing the required urban safety equipment and needs installed per 1501:9-9-05E(4)(b-e). There was an identification sign posted at the tank battery that has been amended with the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence surrounds the tank battery. The 210 BBL tank extends more than 3 foot above the fencing. Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f). There was no landscaping around the tank battery, however the tank battery was located in a wooded area. No gate has been installed at the entrance. The gate was not waived during the pre-site inspection. Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2), and post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f).

PAC needs to: 1) Install the required urban safety equipment on the 210 BBL production tank and needs installed per 1501:9-9-05E(4)(b-e). 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05E(4)(f). 3) Install the landscaping around the tank battery. 4) Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2), and post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f).

The tank battery consists of a 210 barrel production tank, a 50 barrel, poly brine tank, and a vertical, mechanical separator. There is dike around the tank battery. An identification sign is posted. There is no urban safety equipment on the production tank. There are locked, chain link fences surrounding the well and tank battery. The lease road is in stable condition. I called and spoke with PAC Drilling. I instructed PAC to immediately install the proper urban safety equipment on the production tank. Follow-up inspections will be conducted.

My inspection found the well surrounded by a chain link fence and equipped with a plunger lift unit (rabbit). The tank battery was surrounded by a chain link fence and contained one 210 barrel storage tanks, one 50 barrel brine storage tank, the proper identification posted, and a separator. The production equipment had the proper spacing, the pit were closed, and all the debris removed. The site has been graded and seeded to prevent erosion and sedimentation. Inspection passed.
