There is some junk not associated with the well on site, an old boat and several tires which will not be part of the final restoration.
0490465812 2016-06-09 Plug / Plug Back No Violations On the day of my inspection accompanied by inspector Mark Moreland, I discovered the location has been reclaimed. However, no vegetation was established. I will be back at a later date to check on the final restoration.
0879344796 2016-05-09 Plug / Plug Back No Violations Began today with a Safety Meeting that included Atlas Noble, Nicks Well Plugging, Specialized Safety Service and ODNR Supervisors Bear and Moreland along with 3 Inspectors. Went over safety standards and plugging operations. Next went to the well to begin plugging. The first action was to set up a safety perimeter and a first and second muster area. Specialized Safety set up monitors and alarms and then allowed the plugging crew to add Sulfide blocker to the annular space between the 8 5/8 inch casing and the 4 1/2 casing. After the blocker was applied the readings dropped to 1 or 2 PPM. Nicks Well then inserted a spear into the 1 1/2 inch diameter tubing and was able to pull out one joint. The rabbit came up and they think the tubing is clear. Appalachian Well should be able to run in and shoot off the seating nipple at the bottom. They are scheduled for Tuesday morning at 7 AM
0989911503 2016-05-05 Administrative Inspection No Violations Meeting today with Inspector Zollars to calculate top of cement and figure plugs for the upcoming plug job. Prepare plug schedule before the meeting with Atlas Noble and Nicks Well Plugging. Calculate displacement and perforations needed to plug the H2S well and make copies.
1460645743 2016-04-14 Production Wells Violations Noted I am on site accompanied by Inspector Mark Moreland, I witnessed the production tanks have been removed and production equipment consist of a horizontal separator. The well is utilizing a plunger lift system equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, and 1.5 inch diameter production tubing. All valves are in the closed position. Utilizing an ODNR Honeywell PhD6 meter I recorded readings of 100 ppm H2S at the well head. The well head has lost integrity and is venting H2S (Hydrogen Sulfide) gas to atmosphere through a quarter sized hole placed on the side of the well head. It appears all strings of casing/tubing have also been impacted by the poisonous gas, causing the loss of integrity and promoting higher safety/environmental risk. The well has not produced commercially since 2005. After we conducted our inspection, I then placed a call to Pat Ervin (Atlas Nobles Supervisor) at (330) 319-3155 to inform him that this well is presenting an eminent threat to the environment and health/safety. I also stated that the well needs to be plugged as soon as possible. Pat informed me that he would provide me with a written plan and he understood the well was a hazard and needed to be plugged. Pat also stated that he plans to get the well plugged as soon as possible. The well owner/ operator shall apply for a permit to plug the well. The well owner/operator shall submit a written plugging schedule to Joe Zollars at 2207 Reiser Ave S.E New Philadelphia, Ohio 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 60 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1462881548 2016-05-04 Administrative Inspection No Violations I had a call today from Pat Ervin to go over information on the up coming plugging operation for the Sheehan well. Pat said that he wanted at least two Specialized Safety personnel on site for this plug. This is due to the known hazardous H2S conditions at the well. He also advised that he wanted them to have air packs available should any problem arise. I ask Pat if he had plans for how the well was to be plugged and he advised that a meeting was scheduled for Monday. The details will be worked out during that meeting. Atlas, Nicks Well Plugging and Specialized Safety along with ODNR staff are scheduled for 8:30 AM I contacted District Supervisor Mike Moreland and he is to contact the remaining ODNR staff that will attend the meeting.
1901435660 2016-05-19 Final Restoration No Violations I am on site today to check the well for leaks after it has been plugged. I did not find any leaks or gas bubbles. Well has been cut off and area needs graded and seeded. Final could not be passed today.
1913847673 2016-04-21 Production Wells No Violations I am on site today accompanied by my supervisor Mike Moreland following up on Compliance Number 1460645743, were I witness no changes has occurred since my last inspection. The well head is still leaking H2S gas at approximately 125 ppm and should be considered a definite health/environmental issue. With the last conversion I had with Pat Ervin, he stated that Atlas should meet the 60 day requirement to plug the well bore.
2098667333 2016-05-09 Production Wells No Violations On the day of my inspection accompanied by ODNR Southeast Regional Supervisor Brent Bear, Southeast District Supervisor Mike Moreland, Inspectors Mark Moreland and Joe Zollars, we met with Atlas Field Superintendent Chuck Wood and Nicks Well Plugging representatives and discussed upcoming plugging of well.
