| I received a call from Donny Erwin, the toolpusher on Capstar Rig 4 at 19: 30 on 9/9 reporting a strong gas odor at 3517. They were in the process of loading the hole and converting over to fluid drilling. I received no calls from either the public or local officials reporting any problems.

| I was on location to check the status of preliminary restoration for this urban production well that was due in 2010.  A check of records in RBDMS indicates that preliminary restoration was passed on 8/26/2010 by Inspector Bob Worstall.  During my inspection, I found that the lease road gate was locked and the road was in good condition.  The site was graded and grass was well established on the location.  The proper ID was posted at the entrance and at the tank battery.  The tank battery was diked, fenced, locked,  and contained 1-100 and 1-140 barrel stock tanks as well as a horizontal separator and a meter run.  The wellhead was fenced, locked, and operating on a plunger lift system.  The gauge indicated there was 450psi on the well.  A master meter carrying gas from the Wurm wells to the west was recently installed to the west of the tank battery.  Preiminary resoration is approved.  No violations.

0124479306 2010-04-06 Production Wells No Violations The well is in production and the affected area still needs to be graded, seeded and mulched. Preliminary restoration is due later this spring.
0639672294 2009-09-10 Drill / Deepen / Reopen No Violations I received a call from Donny Erwin, the toolpusher on Capstar Rig 4 at 19: 30 on 9/9 reporting a strong gas odor at 3517. They were in the process of loading the hole and converting over to fluid drilling. I received no calls from either the public or local officials reporting any problems.

1047927289 2009-09-18 Drill / Deepen / Reopen No Violations Capstar Drilling has moved off the location. The drilling pits are fenced, lined and pumped down. Solidification is scheduled for next week.
1354117846 2012-11-27 UrbanPreliminary Restoration No Violations I was on location to check the status of preliminary restoration for this urban production well that was due in 2010.
1434827437 2009-10-08 Drill / Deepen / Reopen No Violations The drilling pits are fenced and lined but are still open. I spoke with Cal Marks who said that he intends to solidify and close the pits next week. The 30 day deadline is 10/18.
