0905130921 2018-08-17 Production Wells Violations Noted On this day, I am conducting an inspection in reference to Complaint #5927. Landowner, Connie and Randall Reid expressed concerns of safety with an exposed 1 inch diameter plastic flowline along their drive. They requested that the flowline be buried. I found approximately 125 feet of exposed 1 inch plastic line along the drive. The well is equipped with 4.5 inch diameter casing and 2.375 inch diameter tubing with the production valves open. The well appears to show signs of recent activity. There are (2) 100 barrel steel tanks, but only 1 within an earthen dike. It will be noted that there is a vertical separator near the wellhead, within less than 20 feet, which does not meet the spacing requirements of 50 feet minimum spacing from the wellhead. The identification at the tank is faded and needs updated, and annual production has never been reported. Multiple pictures were taken. An attempt to contact Tri-City Drilling was made on 8/21/2018, and spoke with Greg Nuzum, to inform him of the issues and time frame for remediation. To bring the well within compliance, the following will need to be conducted. 1) Bury the gas line approximately 24 inches 2) Move the separator within 50 feet minimum of the wellhead 3) Properly identify the wellhead and/or tanks.
0996162826 2018-09-26 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection in regard to Complaint #5927. There has been no progress made to remediate the issues and get the well back within compliance. The section of pipeline that was requested to be buried is above ground, the identification is not updated, and the separator has not been adjusted to the minimum 50 setback from the wellhead. Updated pictures were taken. An attempt to contact Greg at Tri-City Drilling was made on 9/26/18, with no answer, and a voicemail left requesting a callback.
1543419519 2018-11-28 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection in regard to Complaint #5927. No progress has been conducted to regain compliance. On 11/28/2018, I spoke with Greg Nuzum of Tri-City Drilling, and he indicated that he has made contact with his contractor, but the contractor has been unavailable due to engagements with other activities. I reiterated that progress needs to be made, and he will need to attempt to commence work to address the issues.

On 4/12/19, I spoke with Greg Nuzum of Tri-City Drilling, and he stated that the weather and health issues had prohibited the work to be conducted throughout the winter months. I explained that progress needs to be made, and he is attempting to contact a contractor. Updated pictures were taken.
