1149117149 2018-08-21 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. The issue is not resolved. The chloride levels within the dike area are still testing over 500 parts per million chlorides with a High Range Quantab. Remediation work still needs to be conducted to keep the dike free of any production fluids. Additionally, there is a PVC drain with a riser pipe installed through the dike, that will need to remain water tight, in order to prevent any type of fluid release. An attempt to contact Paul Valentine was made on 08/22/2018 at 8:10 am, and left a voicemail. I spoke with Paul Valentine on 8/23/2018 and explained the issue that still needs to be addressed.
1368418305 2018-06-27 Production Wells Violations Noted I received an e-mail from Inspector Cynthia Van Dyke that she had received a message from Karen Yoder that it appears there was some kind of spill in the tank battery for this well. I arrived on location at 8 AM on 6/27/18 and found the tank battery described in the e-
1533129058 2018-08-01 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection per request of Southwest Supervisor Eric Conrad. A noticeable attempt of remediation has been conducted, with the initial stages of green vegetation beginning to re-establish along the hillside. The area within the dike appears to have some standing fluid and was tested with a High Range Quantab resulting in 900+ parts per million chlorides. The owner shall address the issue within the dike before the violation will be passed. The well is equipped with 2.375 inch diameter tubing and 1 inch diameter vent packer with the production valves open. It is produced by means of a pumping unit with rods and a gas engine. I spoke with Rob Armstrong and John Thompson during the inspection. Pictures were taken during the inspection. An attempt to contact Paul Valentine was made 08/01/2018 at 11:32am with no answer, and voicemail requesting a callback. Received callback, and spoke with Paul at 4:00pm 08/01/2018, and requested him to address the high chloride issue within the dike.
1576612830 2013-04-02 Production Wells No Violations Owner of wells, Darrell Yoder going by Double D, requested permit numbers for these 3 wells around his house. This well has pumping unit, tank is located on next road over, diked with another tank. Jason, pumper, says he will have identification up as soon as he gets numbers. I called 4/8/2013 and gave numbers to him.
2005525996 2018-09-04 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection in regard to the pollution and contamination issue cited on 06/27/18. The issue is resolved. Vegetation is re-establishing along the hillside and the standing fluid is no longer present within the dike. A voicemail from Paul Valentine on 09/04/2018 indicated that he will be spreading new soil within the dike area. It will be noted that I spoke with Southeast Supervisor Eric Conrad about the issue.
