The well was in violation of 1509 12(B), well shall be plugged. The well consists of a pumping unit and 1-100 barrel tank. I texted well owner Mr. Doug Kitchen at 10:39 AM, he responded with the well will be plugged on or before 7.30.2018.

0353598517 2018-09-28 Final Restoration No Violations I conducted a restoration status check on the well location. I observed two tanks, electric line, and a wellhead at the well location. The well location has not been graded or restored. I contacted Doug Kitchen at 11:14 AM and discussed his plans on when the restoration will be completed. Mr. Kitchen said he is planning on working on it next week and hopes to have it finished. Time of inspection: 10:00 AM.

0413106430 2012-06-28 Production Wells No Violations FOLLOW UP TO NOV # -562326343 Inspection was conducted with Jon Scott South region inspector 3. The first thing that was noticed was contamination in the tank battery dike. Contamination had also escaped the dike and ran down the hill. A scald area 16 feet by 15 feet was observed. There was also a black plastic line that ran from the bottom of the tank over to the well head. It was not connected to anything and a scald area (8 feet by 25 feet) was observed at the end of it. Evidence of contamination was then observed at the well head. Well head was corroded as well as scald areas around the well head. There were two metal plates connected to pieces of casing that were driven vertically in the ground close to the well head. Down the hill, close to the creek was another large scald area that when tested for chlorides with Quantab titrators, completely maxed the Quantab out (greater than 6000 ppm chlorides). The water in creek was also tested with Quantabs in several different places. Chlorides were detected there as well. There is no identification at the well head or the tank battery. All contamination needs to be removed and hauled away from site. New soil needs to be placed in those areas. Identification needs to be placed on the well head and/or the tank. Spoke with Doug Kitchen, Bill Wray, Bill Deible, and Dale Campbell in person about this and gave specific instructions to them about the remediation of the site.
0434954866 2012-07-10 Production Wells No Violations Witnessed Doug Kitchen excavate the well head and expose the 2 steel manifold piping. 2 lines were plumbed to by-pass the separator and flow to the annulus. Took pictures. Doug damaged a 2 gas line and had to shut the valve on Chesapeakes gathering line. Soil samples were taken with Andy Thomas MRI 2. Chieftain hauled one load of contaminated soil to landfill.
0436401236 2012-07-17 Production Wells No Violations Investigation of the contamination on this well site continues. Kyle Tharp, Jake Glascock, Mike Williams, Josh Garland, and Jon Scott were all present. Kyle Tharp ran the EM-31 electrical resistivity meter, Josh Garland, Mike Williams assisted me in taking soil samples, Jon Scott and Jake Glascock assisted Kyle. Nine soil samples were taken, and Kyle generated data with his instrument. Doug Kitchen was also on site witnessing activities and taking pictures with his cell phone.

The well owner Hocking Hills Energy, will be notified if the plugging schedule is approved or denied.
0957786887 2012-07-05 Production Wells No Violations Checked on site. Scott Courter was in the process of removing extra 100 barrel tank. Well head had been dug up and covered back over. This was evident due to the amount of disturbance around the well head, and tracks from a small trackhoe were left behind. Pictures were taken. I had not been notified that there was going to be any activity at the site. Part of the instructions that I had given the company was that I was to be notified if any work was to be done.
0980227237 2015-08-31 Production Wells Violations Noted On the day of my inspection there was no identification anywhere at the site. The pumping unit and well head was overgrown with weeds. The valve was shut on the production line. There was a 100 barrel steel tank approximately 10 feet from the well head. There was a vertical separator and drip attached to the sale line for gas. There was a chart with only one half inch mark on it. The pumping unit motor used electricity and upon further inspection I found the breaker to be in the off position. The breaker box also was home to a very large wasps nest. The 240 volt line is on the ground I was able to follow the cable, on the ground, to the beginning of the lease road across a creek about one half of a mile. There has only been 42 miff of gas production reported to the division the last two years with no oil reported. I contacted Kip Thompson the county inspector and emailed him a copy of my inspection.
1065861359 2008-08-26 Production Wells No Violations A follow-up inspection revealed non-compliance. Bare areas were observed over a hill from both the tank battery and well location. A black plastic flowline was observed to be connected to the bottom of the 100 bbl tank and routed to a location at the top of the hill. At the time of the inspection, the tank was empty and valve closed. The line was not connected to the well. Quantab placed at location at bottom of hill showed 10 units. No motor was observed for the pump jack. Identification was not observed.

1164204604 2012-07-09 Production Wells No Violations With the approval from management, Jon Scott and I dug up the well head where it had been dug up previously. Frequent chloride tests were performed on the soil on the way down. Chlorides were detected down to 4.5 feet and around all underground plumbing. Grass was observed in several spots below the surface down to 4 feet as well as insects that dont live that deep in the soil further confirming that area had been dug up previously to that depth, then covered back over with soil obtained from somewhere else on the site. There was a plug in the 8 casing and in a pipe fitting directly across from the plug in the 8 casing. Both had fresh pipe dope on them.

1342050529 2012-07-09 Production Wells No Violations Follow up inspection with Andy Thomas MRI 2. We excavated a recently disturbed area at the wellhead. Found disconnected 2 annular disposal line. Annulus valve had a 2 plug with fresh thread sealant on the threads. Green grass clods were observed 4 below surface. Well casing has been recently excavated. Soil is contaminated with brine. Took photos. No efforts have been made to remove the contaminated soil.

1342620756 2012-07-10 Production Wells No Violations Witnessed clean-up efforts performed by Chieftain Energy. A large amount of underground plumbing was observed around the well head 4 to 4.5 feet below the surface. Doug Kitchen was operating the backhoe. Several lines were hit and tore out in the process of digging around the well head. One of the lines was the production line. When it was hit, pressure backfed from the sale line and a valve at the separator had to be turned off to stop the flow of gas. Soil samples were acquired from 5 locations at the well site. Accompanied by Inspector Jon Scott.
1344512510 2012-07-24 Administrative Inspection No Violations Had meeting with new well owners. Rocky King, Dan Goins and myself were present on behalf of the state. Dale Campbell, Dave Poling and Doug Kitchen were present on behalf of Chieftain Energy. Rocky had outlined his expectations as far as a producing operation goes and then we talked extensively about cleaning up the contaminated soil at this well site. Bioremediation was talked about as an option for some of the area. In other parts of the area digging and hauling material away was also talked about.

1533818132 2018-08-09 Production Wells No Violations I conducted a compliance notice follow-up on this well today. Inspector Kip Thompson issued a compliance notice for the well to be plugged to Hocking Hills Energy Well Service on 10/4/2017. My inspection revealed that the well has not been plugged. During the inspection I observed that the pumping unit has been pulled from the well and it appeared that the area around the well was cleared to prepare for a service rig to set up on the well. A backhoe was sitting next to the well during the inspection. There were no personnel present at the time of the inspection. I contacted Doug Kitchen at 9:48 AM and discussed his plan for plugging the well. Mr. Kitchen said that a service rig will be moving on the well tomorrow and plugging operations should begin the following week. Time of inspection: 8:20 AM.
1534508624 2018-08-17 Plug / Plug Back No Violations Appalachian well service on site at 9:55 AM. They ran a bond log and determined free pipe on the 8.62 inch diameter casing was at 950 feet. Appalachian Well Service shot the casing off at 959 feet at 10:40 AM. Knox Energy pulled 60,000 pounds on the casing but the casing was not free. I left location at 11:10 AM as I was out of hours for the week.

I told Dan that it was a civil matter. I made a courtesy call to Doug Kitchen and ask Doug to contact Dan Duval regarding this matter.

The 100 barrel tank has been removed and the site has been graded to allow room for the equipment. I completed the cement calculations and displacement volumes for the plug job. I will confirm casing weights with the operator and adjust if necessary.
