The first thing that was noticed was contamination in the tank battery dike. Contamination had also escaped the dike and ran down the hill. A scald area 16 feet by 15 feet was observed. There was also a black plastic line that ran from the bottom of the tank over to the well head. It was not connected to anything and a scald area (8 feet by 25 feet) was observed at the end of it. Evidence of contamination was then observed at the well head. Well head was corroded as well as scald areas around the well head. There were two metal plates connected to pieces of casing that were driven vertically in the ground close to the well head. Down the hill, close to the creek was another large scald area that when tested for chlorides with Quantab titrators, completely maxed the Quantab out (greater than 6000 ppm chlorides). The water in creek was also tested with Quantabs in several different places. Chlorides were detected there as well. There is no identification at the well head or the tank battery. All contamination needs to be removed and hauled away from site. New soil needs to be placed in those areas. Identification needs to be placed on the well head and/or the tank. Spoke with Doug Kitchen, Bill Wray, Bill Deible, and Dale Campbell in person about this and gave specific instructions to them about the remediation of the site. |

The valve was shut on the production line. There was a 100 barrel steel tank approximately 10 feet from the well head. There was a vertical separator and drip attached to the sale line for gas. There was a chart with only one half inch mark on it. The pumping unit motor used electricity and upon further inspection I found the breaker to be in the off position. The breaker box also was home to a very large wasps nest. The 240 volt line is on the ground I was able to follow the cable, on the ground, to the beginning of the lease road across a creek about one half of a mile. There has only been 42 miff of gas production reported to the division the last two years with no oil reported. I contacted Kip Thompson the county inspector and emailed him a copy of my inspection. |

Mr. Kitchen said that a service rig will be moving on the well tomorrow and plugging operations should begin the following week. Time of inspection: 8:20 AM. |
