Nicks pulled the tubing from the hole, attempted to pull 6.25 casing. The casing parted approximately 24 down hole. They ran 2.375 tubing back in to 160 and mixed cement until it circulated to surface. Nicks cut the well head off 30 below grade, tacked on a plate inscribed with ODNR - 0003, drained and closed the pit, moved all equipment off of location, graded out topsoil to approximate original contour. See plug report for further detail. |

I called Gonzoil Inc, 330-497-5888 and left Doug Gonzolous a message on his phone. The next was Bishop Well Ser. 330-264-2033 and left a message on there phone. At 6:00PM I called Jeremy about what is taking place tomorrow with the Corp. and Doug Gonzolous called me back to make an appointment at the well site from Friday morning at 8:00AM, 4/15/11. |

The Sanitary System call came from Jeff Pinkey(330-716-1646). The report was that this well was running into a water way. I called Jeff Pinkey to see where this well is. He stated to go to #103 gate on Palmyra road and then go south over steel bridge, west to the high lines, south to a clearing, southwest to the well in the woods. I asked if he could meet me tomorrow to show me the well. He was busy. I told him I would go out tomorrow. |

Both sets top were peeled back. Oil was burping up 10 inches down from the top. I called Jay Cheslock for advice on this matter. The well file was downloaded from Columbus. The well was drilled in 4/15/1936. Total depth was 410. There were no records of the casing run. This complaint continues. |

I stated that the well liquid could get into the Meander Creek and then into Meander Lake which feeds Youngtowns drinking water. I will keep him posted on the process of plugging this well. |

On the day of my inspection I found that all oil field related materials and debris were removed from the site. All equipment and materials related to plugging the well were removed from the site. The soil was graded out to the approximate original contour with a small drainage ditch in place per landowner request. Emergent understory vegetation was established in the disturbed forested area, and short grasses were established along the disturbed forest edge. There was no lease road associated with this well. The landowners drive path was used to access the well. The path sustained no damage during the course of plugging operations. This site passes final restoration. |
