0053510738 2011-03-17 Production Wells No Violations I went to houses on Gault road to try and find out who owns this property. I went back to house #5303. But no one was home. This complaint continues.
0386826085 2012-04-24 Orphan No Violations I was on location to witness orphan well plug job. Nicks Well Plugging hooked up to 2.375 tubing, pumped cement plug from total depth of 410 to 160. Nicks pulled the tubing from the hole, attempted to pull 6.25 casing. The casing parted approximately 24 down hole. They ran 2.375 tubing back in to 160 and mixed cement until it circulated to surface. Nicks cut the well head off 30 below grade, tacked on a plate inscribed with ODNR - 0003, drained and closed the pit, moved all equipment off of location, graded out topsoil to approximate original contour. See plug report for further detail.
0689779031 2011-03-10 Orphan No Violations I received a call from Tom Hollaway of the Mahoning County Sanitary System(330-652-8614)about this well. He wanted this well taken care of. I stated that we are going to install a dike around the wellhead. I also told him to call Jay Cheslock . Chris Grimm and myself returned to the wellhead. We diked around the wellhead and installed diapers inside the dike. I took more pictures. This complaint continues.
0823999432 2011-08-11 Orphan No Violations Chris Grimm and myself, Carl Roberts met with Mike McCormac and Terracina Maxwell at Gault road. We all inspected the well and talked about the way to get this well plugged and the access to this orphan well. There were no violations at this time.
1063491801 2011-04-12 Orphan No Violations I met with Rick Simmers at the Uniontown office about this well. He directed me to call the Corp of Engineers to make sure that the well area were not in wetlands. I called the Pittsburgh office 412-395-7155 and talked with Matt Mason.He is going to be in the Youngstown area tomorrow, 4/13/11 and would call me for an afternoon meeting. I called Jeremy to let him know. I called 3 bidders, Moores Well Service Inc. 330-650-4443 and left Jeff Moore a message on his answering machine. I called Gonzoil Inc, 330-497-5888 and left Doug Gonzolous a message on his phone. The next was Bishop Well Ser. 330-264-2033 and left a message on there phone. At 6:00PM I called Jeremy about what is taking place tomorrow with the Corp. and Doug Gonzolous called me back to make an appointment at the well site from Friday morning at 8:00AM, 4/15/11.
1299787692 2011-03-08 Production Wells No Violations At 4:00PM I received a call from Bob Wentzell(330-207-2048) about a well that the Mahoning County Sanitary System called him about. The Sanitary System call came from Jeff Pinkey(330-716-1646). The report was that this well was running into a water way. I called Jeff Pinkey to see where this well is. He stated to go to #103 gate on Palmyra road and then go south over steel bridge, west to the high lines, south to a clearing, southwest to the well in the woods. I asked if he could meet me tomorrow to show me the well. He was busy. I told him I would go out tomorrow.
1299841600 2011-03-09 Production Wells No Violations I investigated the complaint. The wellhead was GPSs. It was in the woods. The 8 5/8 casing was open as well as the 5 1/2 casing. Both sets top were peeled back. Oil was burping up 10 inches down from the top. I called Jay Cheslock for advice on this matter. The well file was downloaded from Columbus. The well was drilled in 4/15/1936. Total depth was 410. There were no records of the casing run. This complaint continues.
1300387972 2011-03-16 Production Wells No Violations Went to house #5303 on Gault road. Talked with a Mrs. Lori Hornberger about the ground east of their house. She stated that there are no wells on their property. I told her that this well was drilled in 1936. I gave her a Mapquest color map of the area. She would ask her husband about this well and if is on their property. This complaint continues.
1300803526 2011-03-18 Production Wells No Violations I went out to try to find the owner of the property. Jeremy and Lori Hornberger were not at home. I went next door and talked with Ray Minnitti of 5313(330-538-3061) Gault road. He stated the Jeremy works until 6:00PM. I will try and call Jeremy(330-538-0456) at that time.
1301510170 2011-03-29 Orphan No Violations I called Jeremy Hornberger(330-538-0456) of 5303 Gault road about going onto his property to get to the well. There is a lease road to another well close by. I will give him the form to fill out about his property. Jeremy agreed to meet with me on Thursday, 3/31/11 at 6:00PM. I will be there.
1301597937 2011-03-31 Orphan No Violations I met at 6:00PM, 3/31/11 with Mr. Jeremy Hornberger of 5303 Gault road, home(330-538-0456)cell (330-720-6255) about showing him the orphan well that is on his property. We inspected the orphan well and I also gave him the release form to fill out and the Orphan well brochure. Mr. Hornberger wanted to have an attorney look over the Orphan well release form. He was not real happy about having this well plugged. The well is 1/4 mile off the 5303 Gault road. The lease road crosses Meander Creek. I stated that the well liquid could get into the Meander Creek and then into Meander Lake which feeds Youngtowns drinking water. I will keep him posted on the process of plugging this well.

1302116218 2011-04-06 Orphan No Violations I inspected the fluid level inside the casing. The fluid level was down 10 inches from the top and the dike area was holding.

1302777623 2011-04-13 Orphan No Violations I met with US Army Corp of Engineers, Matt Mason, Alex Kostra and Ohio EPA Ed Wilk at the house of 5303. We walked down the private road to Meander Creek. The creek was too deep so we went in from the north side. Mr. Mason stated that a Nation Wide Permit is necessary. It is a maintenance type permit. Mr. Mason will email me the application with an explanation of his findings.
1338312864 2012-05-29 Final Restoration No Violations I conducted an inspection on the status of the final restoration of this orphan well site. On the day of my inspection I found that all oil field related materials and debris were removed from the site. All equipment and materials related to plugging the well were removed from the site. The soil was graded out to the approximate original contour with a small drainage ditch in place per landowner request. Emergent understory vegetation was established in the disturbed forested area, and short grasses were established along the disturbed forest edge. There was no lease road associated with this well. The landowners drive path was used to access the well. The path sustained no damage during the course of plugging operations. This site passes final restoration.
1646665868 2012-04-23 Orphan No Violations I was on location to witness orphan well plug job. Nicks Well Plugging moved equipment onto location, rigged up, dug and lined pit, welded on an 8.625 well head, ran 2.375 tubing to total depth, packed off the well head, shut down for the day. See plug report for details.
1830802278 2011-08-16 Orphan No Violations I met with Mr. Jeremy Hornberger and Nick from Pluggers about viewing the orphan well. We inspected the open casing and the area around the well. There were no violations at this time.
