0341548025 2008-04-14 Final Restoration No Violations Culvert has been removed from drainage ditch. Sediment has been removed from ditch where erosion/sedimentation occurred. Ditch slope has been graded back to original slope. Dirt used to cover culvert is piled next to hole where removed. Hole needs to be backfilled and ditch bank seeded.
0359602506 2008-01-08 Orphan No Violations Checked site for final restoration. A culvert and dirt was placed in ditch along Warner Road to access the farm field to plug the well. The culvert and dirt are to be removed but has not yet been done. Erosion occurred in the dirt filling over the culvert. The material was deposited in the ditch downstream from the culvert. Spoke to Bob Harpster by phone. He said they are waiting for ground to freeze to complete work which includes filling a hole in the farm field where the dirt was removed for the crossing. Dirt deposited in ditch from erosion must be removed when culvert/dirt are removed. Bob said he would notify inspector when work is completed. Picture taken.
1193862050 2007-10-31 Orphan No Violations Call from Bob Harpster that drilling on steel in hole at depth of approximately 120 feet. When arrive on site drill had advanced 9 inches in 5.5 hours. Since this is a 6 inch hole Bob thought that if it was a cannonball that it would be drilled thru by now. After about another hour of drilling it appears as though they drilled thru the steel obstruction. Operator will return tomorrow to continue drilling. There was no casing in the well to a depth of approximately 90 feet.
1193941641 2007-11-01 Orphan No Violations Call from Bob Harpster that obstructions drilled thru at 120, 160, 340. When drilled through obstruction at 340 feet, oil and gas were released. Another obstruction was hit at 374 feet. Since there is no casing in hole from about 20-100 feet (limestone), the fear was that drilling through the next obstruction may release more oil and gas that could contaminate the ground water in the zone with no casing. At that time the landowner from 5022 Warner Road (about 600 feet NE of the well) reported that water was discharging from the top of their water well. This was confirmed by inspector with landowner. Bob Harpster said approximately 1600 gallons of water had been used during drilling. No additional drilling will be done and well will be plugged at depth of 374 feet.
1194010605 2007-11-02 Plug / Plug Back No Violations Operator ran 350 feet of 1.5 inch tubing into well for plugging. It was thought that the tubing ran inside the 160 feet of 2 inch tubing that broke off in the hole. Well was cleaned out to depth of 374 feet. Cement pumped into well should have gone thru 2 inch tubing and fill well thru bit opening at bottom of well. Checked water well at 5022 Warner Road after plugging and no water was noted discharging from the well.
1206388276 2008-02-28 Orphan No Violations Culvert in ditch has not yet been removed. Erosion continues on the ditch bank and sediment noted in ditch. Bob Harpster said contractor was hired to do work but it has not yet been completed.
1211483997 2008-05-21 Final Restoration No Violations Vegetation is growing along the banks of the drainage ditch. Hole in field has been fiiled and the field is planted in crops. Final restoration is acceptable.
