0624081959 2015-08-03 Casing String Witnessed No Violations Stopped by drilling rig to inspect 9.625 casing string and talk to company personnel about upcoming cement job.

Patterson rig 296 they were at casing point@8400 and plan on running 8350+or- casing.

1062692051 2018-08-08 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 610 pounds per square inch, the 13.375-inch diameter surface casing gauge read 190 pounds per square inch. Production equipment included an individual Gas Processing unit and 6- 400-barrel production fluid tanks shared with 34013209050000, 34013209030000, 34013209020000. The well pad and all equipment were contained in an earthen dike, the dike had been disturbed by midstream on the western edge of the pad. There was secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag was present, and production was up to date. There were three wells also on location that were in completion process with Mountain State Pressure Systems snubbing rig was currently on the last well to have isolation plugs drilled out.
1077965843 2015-01-21 Casing String Witnessed No Violations On the date of inspection I was on site to witness the grout job for the 24 inch diameter surface casing. ODNR Inspector Andrew Corder was on site earlier in the day to witness the surface job but there was a small crack in a weld job near the well head that opened up during displacement. Rice Energy consultant, Greg Ritter, informed me that they had stopped the job and allowed the cement to balance itself out while they corrected the welding problem. Ritter was able to tag cement at 63 feet below ground using a string and weight. Ritter waited for four hours before grouting the backside of the 24 inch pipe to surface. Baker Hughes pumped 100sk of Class A, at 15.6ppg. 6bbl of cement returned to surface and stayed at surface while I was on site. Rice will wait 4 hours from pump time to run pressure test.
1245989054 2017-09-25 Production Wells No Violations I responded to a spill that occurred on this pad over the weekend. While a dumpster on site was being emptied the contractor noticed black water that had been spilled from the dumpster during the emptying process. The company screened the location and found elevated conductivity readings. They removed two roll off boxes of material and replaced it with new material. The material that was removed was sent to a landfill for disposal. The out-flow drains were recommended to be closed so that future rain events can be tested before releasing.
1367366592 2018-08-08 Production Wells No Violations On site inspection accompanied by ODNR Inspector Ryan Armstrong. Currently there are 4 producing wells on the pad and 3 new wells that the company has service crews on location and completing. The said well is equipped with a lower A section and upper B section and is producing off the 5.5 diameter production casing. The company has installed 2 diameter riser pipes in the cellar, the pipes are connected to the A&B section well head and are equipped with valves and gauges to allow for annular pressure monitoring. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 170 pounds per square inch (psi). The B section which monitors the pressure between the 9.625 and 5.5 diameter casing strings shows 610 psi on the gauge. The 5.5 diameter production string internal pressure is 1,650 psi and the production flow line pressure is 1,128 psi. There is currently cages placed over the 4 producing wells and barriers around them. The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. At the rear of the well pad a midstream operation has been installed. On this day I contacted ODNR Belmont County Engineer Tech Anthony Carson and informed him that the well pad perimeter earthen berm has been removed in the area of the mid stream operation. Mr. Carson will address the issue with the company. I also spoke with the lease operator who manages the well production on site and requested they address the annular pressure on the # 4 well and post proper identification on the # 8 well.

1421939098 2015-01-14 Casing String Witnessed No Violations On the date of inspection I was on site to witness the cementing of the conductor. I was informed by Rice Energy consultant Fred Rice that Well Service Group had drilled a 36 inch diameter hole to 100 feet. They set 100 feet of 106lbs/ft, 30 inch diameter conductor pipe on bottom. Allied Oil & Gas Services attempted to establish circulation with fresh water before pumping cement and their pump truck pressured up. The job was temporarily shut down while the crane on site lifted the casing high enough for Allied to establish circulation. The crane then lowered the pipe until it washed down to bottom. Allied pumped 331sk of Class A cement, 15.6ppg, 1.19 yield. 3bbl of cement returned to surface.

1517568110 2015-01-21 Casing String Witnessed No Violations On this day the 24 casing was cemented into a 28 open hole to a depth of 317. Baker ran 513 sks of Class A cement with an excess of 30%. Cement to surface was established, however a weld on the top plate was compromised and a leak was noted. The leaked cause the cement to drop on the backside. The cement will balance out and a top off job will be preformed to ensure correct cement bond between the open hole and the 24 casing. No violations noted.

1611207756 2015-01-21 Casing String Witnessed No Violations Cement on the backside of the 24 casing string was tagged after waiting a few hours for it to set up. Cement was tagged at 60 down the backside. Baker then remediated by grouting Class A cement from the point of 60 to surface. No violations noted.
1628177235 2015-01-26 Drill / Deepen / Reopen No Violations Current operations are rigging down to transit off location.
1706675037 2015-08-05 Casing String Witnessed No Violations Witness cementing of 9.625 casing Halliburton cemented 8360 of casing in place with 1290 sacks of lead cement with a weight of 13.4# density and yield of 2.53 and 400 sacks of tail with a weight of 15.6# per gallon and yield of 1.29 and they had approximately 94 bbl. of returns.
1817920871 2020-2-21 0:00: PW No Violations 0.20
1848616533 2018-05-24 Production Wells No Violations On site inspection found there is a total of 4 wells in production on the well pad and 3 additional wells that are being stimulated. The 4 wells have steel cages over them and concrete barriers around them. Annular pressure monitoring. The tank battery is on the well pad and consists of 6 steel construction 400 barrel capacity tanks situated inside a polymer lined steel containment area. The company has annular pressure transducers monitoring the annular pressure on the 13.375 and 9.625 well head sections.
