1062692051 | 2018-08-08 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with a four-string casing design with 5.5-inch diameter production casing, and 2.875-inch diameter tubing that appeared to be open flowing. Annular pressure monitoring was in place, the 9.625-inch diameter intermediate casing gauge read 610 pounds per square inch, the 13.375-inch diameter surface casing gauge read 190 pounds per square inch. Production equipment included an individual Gas Processing unit and 6- 400-barrel production fluid tanks shared with 34013209050000, 34013209030000, 34013209020000. The well pad and all equipment were contained in an earthen dike, the dike had been disturbed by midstream on the western edge of the pad. There was secondary containment around the production tanks. There were multiple Emergency Shutoff Devices on location, Identification tag was present, and production was up to date. There were three wells also on location that were in completion process with Mountain State Pressure Systems snubbing rig was currently on the last well to have isolation plugs drilled out. |

ODNR Inspector Andrew Corder was on site earlier in the day to witness the surface job but there was a small crack in a weld job near the well head that opened up during displacement. Rice Energy consultant, Greg Ritter, informed me that they had stopped the job and allowed the cement to balance itself out while they corrected the welding problem. Ritter was able to tag cement at 63 feet below ground using a string and weight. Ritter waited for four hours before grouting the backside of the 24 inch pipe to surface. Baker Hughes pumped 100sk of Class A, at 15.6ppg. 6bbl of cement returned to surface and stayed at surface while I was on site. Rice will wait 4 hours from pump time to run pressure test. |

While a dumpster on site was being emptied the contractor noticed black water that had been spilled from the dumpster during the emptying process. The company screened the location and found elevated conductivity readings. They removed two roll off boxes of material and replaced it with new material. The material that was removed was sent to a landfill for disposal. The out-flow drains were recommended to be closed so that future rain events can be tested before releasing. |

Currently there are 4 producing wells on the pad and 3 new wells that the company has service crews on location and completing. The said well is equipped with a lower A section and upper B section and is producing off the 5.5 diameter production casing. The company has installed 2 diameter riser pipes in the cellar, the pipes are connected to the A&B section well head and are equipped with valves and gauges to allow for annular pressure monitoring. The A section which monitors the pressure between the 13.375 and 9.625 diameter casing strings shows 170 pounds per square inch (psi). The B section which monitors the pressure between the 9.625 and 5.5 diameter casing strings shows 610 psi on the gauge. The 5.5 diameter production string internal pressure is 1,650 psi and the production flow line pressure is 1,128 psi. There is currently cages placed over the 4 producing wells and barriers around them. The production equipment includes 4 gas processing units and 2 glycol heaters. Upon entering the location on the left side of the well pad is the production fluid facility consisting of 6 steel construction 400 barrel fluid capacity production tanks situated inside a polymer lined steel containment dike area. At the rear of the well pad a midstream operation has been installed. On this day I contacted ODNR Belmont County Engineer Tech Anthony Carson and informed him that the well pad perimeter earthen berm has been removed in the area of the mid stream operation. Mr. Carson will address the issue with the company. I also spoke with the lease operator who manages the well production on site and requested they address the annular pressure on the # 4 well and post proper identification on the # 8 well. |

Allied Oil & Gas Services attempted to establish circulation with fresh water before pumping cement and their pump truck pressured up. The job was temporarily shut down while the crane on site lifted the casing high enough for Allied to establish circulation. The crane then lowered the pipe until it washed down to bottom. Allied pumped 331sk of Class A cement, 15.6ppg, 1.19 yield. 3bbl of cement returned to surface. |
