|
On the day of my inspection I found that Poulson Drilling was rigged up and spudding the well. The permit was on location. The drilling pits were lined and there were no leaks or spills. Poulson was drilling a 15" conductor hole on fluid.

On the day of my inspection Petroset pumped 180 sack s of class A cement down the annulus of the 8.625" and 11.75" casing. The cement never came up to surface. At this point John Bernhardt was instructed to have run a bond log on the hole. Appalachian Well Surveys revealed that the cement top was at approximately 208'. The survey showed good cement bond below 208'. John Bernhardt was advised to let the cement set for 8 hours and make another attempt at filling from the surface.

On the day of my inspection I found that the well site was only 40% to 50% covered in grass. This site does not pass preliminary restoration at this time.

On the day of my inspection I found that PDC Drilling had reached a total depth of 3649'. They were in the process on tripping out of the hole and preparing to run 4.5" casing. The permit was on display in the dog house, there were no sags or tears in the pit liners and there were no leaks or spills.

On the day of my inspection I found that the well head was equipped with a pumping unit. The tank battery consisted of four 100 barrel stock tanks, one 30 barrel poly brine tank, two separators and an earthen dike. Proper identification was posted and I found no leaks or spills. This well shares a tank battery with permits 7116 and 7142.

|
On the day of my inspection I found that Hall Well Service was on location to tag the plug. John Bernhardt reported that the latch-down plug did no seat properly during the cement job. The was found to be at the correct depth. All of the drilling equipment had been moved off of the site. The pits were still open; the fluid level was down approximately 3 to 5 feet and there were no sags or tears in the pit liners.

On the day of my inspection I found that Petroset pumped 35 sacks of class A cement down the annulus of the 8.625" and 11.75" casing filling it to surface. The cellar was also filled with cement above the top of the 11.75" casing. After monitoring the situation for 30 minutes the annulus was still full. The cement was not observed retreating down the annulus. PDC Drilling was cleared to connect the blow out preventer, conduct the pressure test and resume drilling.

On the day of my inspection I found that PDC Drilling reported loosing circulation at 197'. They switched to fluid drilling and drilled an 11" surface hole to 464'. Then they ran new 20# 8.625" casing equipped with six centralizers. They mixed three pallets of mud and 13 bags of cotton seed hulls in an unsuccessful attempt to re-establish circulation. Petroset pumped 10 barrels of fresh water, 300 lbs gel, 300 lbs cotton seed hulls, 75 lbs Cell-o-flake, and 75 lbs of paper lost circulation material followed by 250 sacks of class A cement. Circulation was never established. John Bernhardt was advised to let the cement set up for 8 hours and attempt to fill the annulus from the surface.
