-1774817887 2013-07-25 My inspection found a new dike built around the production tank per my request.
-210484599 2013-02-13 My inspection found the well equipped with a plunger lift unit. The tank battery contained one, 100 barrel production tank. It was surrounded by a weathered dike and not capable of meeting containment requirements. The heat treated separator had the correct spacing and had a 7 day production chart. The lease road was in good condition with a locked gate at the entrance. It's shared with permit number 2-2780. This well was placed into production prior to House Bill 278. The production record were up to date on our database. I called Doug Trevis at 330-544-1165. I instructed him to have a need dike built by 2/22/2013.
