217228777 2012-10-01 I conducted an urban production well inspection status check. My inspection found the well equipped with a plunger lift unit with an approximately 18" diameter oil spill. The tank battery was surrounded by a chain link fence and contained three, 100 barrel oil storage tanks, one, 50 barrel brine storage tank, two separators, and the proper identification sign. I noticed no visible spills or leaks. The lease road and tank battery are shared with permit number 2-3344. I called Don Nething at 330-283-5758 and asked for the oil spill to be cleaned up and to install an identification sign at the wellhead. I will conduct a follow-up inspection by 10/8/2012.
256196892 2012-10-31 I completed a follow-up inspection. My inspection found the proper identification sign posted at the wellhead, the oil leak cleaned up, and new gravel spread around the wellhead.
