-1048460020 2012-08-13 Follow up restoration work has not been completed. Areas around well head, under rig, and under cement pump are still void of vegetation. Small areas of soil in these areas still appear to be contaminated. Lime has been applied to the soil.

-157658114 2012-05-23 Meadows service rig on the well. Sediment fence has been installed between well and Ohio river. Installed containment area made of staked down straw bales and plastic sheeting (down slope from the well). Rig crew pulling rods at 8:30 am. Pulled 1528 feet of rods at 10:00am. Well is deeper than records indicate. Well pressured up and blew out 100 feet of rods and the pump. No one was injured. Blow out created an oil mist that coated Route 124 with oil mist. Emergency responders (Racine Fire Department and Meigs EMA) were called to close the road for safe clean up of the oil. Meadows crew cleaned the road with oil absorbent. Route 124 was open at 1:00 pm. ODOT county manager Brett Jones and State patrol trooper Webb were satisfied with the clean up. Landowner across Route 124 (Dave Wilson's property) was impacted by the over spray. Mist was seen on 3 trees and on a truck and car. Meadows paid Wilson $100.00 to have the vehicles cleaned. 5 3/16" casing head parted 2 feet below surface. 2" tubing has multiple holes in it. Installed 3" casing with a sand hitch ( 3" flared on end and material is wrapped around casing and secured. 3" is lowered into 5 3/16" casing and sand is poured into annulus) tension was pulled on 3" casing and pulled stretch on the 5". 3" by 2" swedge with 2" valve was installed.

Used 60 barrels fresh water to flow oil into pit. Cleaned out annulus and installed wellhead on 10" casing. Ran 1" tubing inside 2" tubing to total depth (1640'). Moved cement pumping equipment on location.

-817261988 2012-05-25 Two water trucks on location. 1640 feet of 1" tubing ran inside 2" tubing. Pumped 1 barrel fresh water at 200 psi. Pressure dropped to 100 psi then pressured back up to 200 psi. Bleed pressure off and connected 1" tubing with valve to Water truck. Well kicked off and flowed into water truck. Pumped 1.25 barrels fresh water through 1" tubing and well kicked off at 350 to 400 psi. Loaded well casing with brine. Circulate brine to surface and into to steel tank. Well is under control. Pumped 5 sacks Cemex cement down 1" tubing. Displace with 1.25 barrels water. Circulated back to surface while pumping cement. Extract 1" tubing. No gas or oil circulated out of well. Shut in wellhead with swedge and valve.
1256366454 2012-07-25 The pits have been closed, and there is no equipment on site. However the soil around the wellhead appears to contain oil and grass has not grown in it as of yet. The landowner states no grass seed was ever put down.
1320111772 2011-10-05 I&O Complaint inspection. Inspector Josh Garland and I met with James Weston at the well site. Well is equipped with 14" , and 10" casing both are open. 5" casing with 2" tubing. Well is not connected to production equipment or production lines. Well is leaking gas in the 10" and 5" annulus. Oil residue is on the ground around the well casing. James Weston told us that when the Ohio river floods and fills up the well that oil is released from the well into the river and onto the property. Photos were taken along with a sketch of the site. The well is being referred to the Orphan well program.
1337870072 2012-05-23 1653 feet of rods were pulled from the well. Records show the well as producing from the Cow Run formation, this well is producing oil from the Berea Sandstone. As the last rods were being pulled, the well kicked and blew trapped gas and bring out of the tubing. The river was unaffected, the contamination was carried towards St Rt 124 by the wind. The well blew itself down. The 5" casing head was completely rotted away and we were unable to install a swedge on that string of casing. So a joint of 4" casing was installed inside the 5" casing with a sand hitch so a swedge could be installed to control the well.
1338492242 2012-05-31 Five inch casing was shot at 777 feet. Unable to pull the five inch. Pumped an 18 sack, 14 lb/ft, class A cement plug from 848 feet to 700 feet. Plug displaced through 1.5 inch tubing with 1.5 Bbl fresh water.
1338569706 2012-06-01 Cement top inside 5 inch tagged at 685 feet. Casing shot at 552 feet, then 441 feet, and 332 feet. After each shot an attempt was made to pull the casing. Each time 20,000 lbs of stretch was pulled and the casing never parted. A 45 sack Class A cement plug was pumped down 1 1/2 inch tubing into the 5 inch casing from 685 to 300 feet.
1339002215 2012-05-29 Damage to neighbors trees across the street was accessed. 1.5 inch tubing was to be used to pump plug between 2" and 5" from 1344'-849'. Cement was tested and the mixture was too thin. The plug was displaced out of the hole. The 5" began to gas so salt water was used to kill the well.
1339161253 2012-06-04 Tagged plug set by Josh Garland, plug at 283'ft. Sot off 5" casing at 243'ft, pulled casing. Ran in 1 1/2" tubing and set plug from 283'ft to 210'ft. Will not remove 10" casing, it appears to be drive pipe and set at 122'ft.
1339682401 2012-05-30 Meet Dan Goins MRI 3 on location and reviewed well records and time line of plugging up to today. I was called out to Gallia County at 9:30 AM, and Dan witnessed the cement plug through 1" tubing from 1344' to 848' 28 sacks. I returned to plug site at 12:30 PM and no gas was observed at the wellhead. Meadows removed the 8" by 4" swedge and ran a 2" overshot onto the 2" tubing. Rig up wire line to shoot the tubing at 824' with a Primer Cord shot. Extract 824' of 2" tubing. Ran collar locator on 5" casing. Collar locator having interference with 2" tubing at 824'. Good collar on 5" casing located at 793' and 771'.
1414407720 2011-12-12 James Weston called and requested a letter from ODNR / DOGRM stating that the well is in the Orphan program and that the state will plug the well. His bank requested it. He also wants a copy of the right of entry form. Request was forwarded to I&O section.
1416454701 2012-05-29 Tag top of cement plug inside 2" tubing at 848 feet. Fluid level at 175 feet. No gas show. Install 8" by 4" swedge. Run 1" tubing inside 5" casing beside 2" tubing to 1344'. Water truck on location. Mix 28 sacks cement for a 496' plug. Circulate water and oil prior to pumping cement. Pumped cement. Cement mix was light. Mud scale weight 11 pounds. Well started to kickoff. Circulate light cement out of well with 30 barrels water until clear. Hauled 35 barrels brine and loaded the well to gain control.

Well located near St Rt 124, Ohio River and houses. Set at plug from 1344'ft to 848'ft between 5" casing and 2" tubing. Mixed 28 sacks of cement for plug, pumped plug through 1" tubing.
