-144590832 2011-04-11 Follow up to inspection #1302011463 .Weak areas of dike had been strengthened and a plastic liner had been added to dike to prevent seepage. Defective tank had been replaced. Contaminated soil area did not appear to have been reseeded yet.
-1792128643 2011-10-31 Bare area by dike still has not been reseeded. Belt from engine to pump jack broken. Called office to let them know.
-1873043741 2011-08-23 Compliance follow up. Dike in good shape. No evidence the ground has been reseeded or mulched. Called Bob Adams of ANG and informed him of continued violation.
-193701358 2012-05-10 I met with Harmon well service who will be performing the work on the dike at the tank battery. We discussed the removal of contaminated soils as well as repair and/or reconstruction of the dike to ensure it does not allow fluids to leak through.
-1937153077 2012-05-07 A large area, roughly 50 square feet beside the dike, was void of vegetation. A field test was administer to search for the presence of Chlorides and the soil in the areas was found to have 4256 ppm Chlorides. A small seep was noticed on the West side of the dike and the area was found to have 336 ppm Chlorides. The areas below the seep was void of vegetation as well. The pit liner used in the dike was compromised in at least on place. The drain had a slow drip, the water dripping from the drain had 664 ppm Chlorides. All contaminated soils shall be removed and disposed of properly. Affected area shall be reseeded and mulched, and steps shall be taken to encourage vegetative cover. Earthen dike shall be repaired in a manner that does not allow the escape of fluids. Dike drain shall be repaired or replaced. 9-1-07

-194489422 2012-05-07 Accompanied Inspector Josh Garland on inspection of tank dike and brine contamination.
-1971869077 2012-05-11 Dike being repaired.
1302011463 2011-03-25 Upon inspection dike around well was found to be full of brine and a seep had developed on the lower side of the dike. It was difficult to tell where the brine had originated. Brine had escaped and contaminated a large area roughly 35'x45'. A high end Quan-tab was used and the brine outside of the dike registered a 3.6 reading, even after a recent rain event. Bob Adams from American Natural Gas was called and a truck was immediately sent to drain the dike. The affected soil in the area must be removed, reseeded, and mulched. NONE(B)
1328541346 2012-02-06 The bare area has still not been reseeded. I spoke with Bob Adams about getting this area reseeded.
1336161862 2012-05-04 Area outside of dike still bare.
1337609484 2012-05-21 The repairs to the dike have been completed. All the contaminated soils have been removed. The drain has been replaced. Vegetative cover not yet established.
1343816489 2008-02-22 An inspection found the well shut-in and incapable of production. The bridle and motor were missing from the unit. There was a gulley, over a foot deep, that ran the length of the lease road. Water was pooling inside the tank battery dike. 9-9-05(A)9

1882725500 2011-03-30 Follow up to inspection #1302011463 . Upon inspection dike was found to be full of brine again. I spoke with BJ Daniel of American Natural Gas and truck was brought in to pump out dike. After the dike had been pumped again it was noticed a brine tank was leaking, the tank was emptied and its contents were moved to a good tank. An extension was granted to make corrections to the dike and restorations to ground.
1930401839 2012-05-16 No further work has been done to the dike.
286985513 2012-05-15 Contaminated soils have been removed from inside and outside of dike. The dike has not yet been fully repaired, but is capable of containing fluids at full capacity.
