-1511995788 2012-07-11 My inspection today found: The salt water injection well within it's allowable limits. A hammer union, before the well head, was dripping brine. The annulus has two valves on it. The first valve is a 2 inch ball valve and the second valve is a 1/4 inch ball valve. The 1/4 inch ball valve leaks. An absorbent pad was placed under the valve after testing. There is a barren area south of the well, the area is approximately 50 feet by 100 feet. The barren area is from the well being worked over when the soil was soft. Brine scalding is evident in the barren area and there is iron and salt staining. There is wildlife tracks and areas where they have scrapped the ground on the barren area. The loading pad and sump had standing water in it. The water in the loading pad tested over 4000ppm chlorides. I contact J.R., the well tender, and advised him to lime, fertilize and seed the barren area. The 1/4 inch valve needs a plug, a needle gauge installed on it or replaced. Loading pad needs water removed from it. Per J.R., he was planning on bringing brine truck over this day to unload the pad. The land owner walked over and meet with me after the inspection. He asked about seeding and repairing the area around the wellhead. I informed him that it's being addressed. The land owner asked about constructing a pond behind the tank battery. I informed him that he should speak to the company and Call Before You Dig.

1242700925 2012-03-28 My inspection of this well site found the well in operation and the pressure on the annulus was 0 PSI. The injection pressure was recorded using a Division of Oil and Gas gauge. Pressure fluctuated between 700 and 860 PSI. The gauge needle was bouncing to the point an accurate reading could not be obtained. When I shut the valve, the pressure on the gauge was at 860 PSI. There was standing water around the well head which I tested for Chlorides and registered 97 PPM. The Chlorides are residual from a well workover conducted over the winter. There was standing water behind the south end of the tank battery covering a brine scald. I tested this water for Chlorides and it tested at 6808 PPM. It appears the well tender was siphoning the dike free of both standing water and released brine. I contacted JR, who tends the well, and I informed him that the well is at it's maximum pressure, to shut down and check pressure. The brine scald area outside of the tank battery needs removed and the area reclaimed. He stated that he was on his way and that the switch on the pump may need changed. I notified him that these missues must be addressed immediately.

1309199044 2011-06-27 Well is operating. Operator has two of the 210BBLs in the battery cut and cleaned.. Two 210 BBl tank shave been painted. Two new tank on site. Spoke to JR, they will be working over the battery after the holiday. New stone for the floor of the battery and new tanks. New identification signs posted at the entrance of the site, at the battery and on the wellhead. Unable to take injection pressure, lock was frozen. Contacted JR, he will replace lock. Operator mowed lease road and area around well. Dike is in good repair, operator will be brining a load of soil in to cover exposed sections of the liner.

Valves had threaded plugs in them.

Operator can discharge water. Tank battery is in good repair. Identification sign is posted. Off loading pad has been steam cleaned. Sump is clear and able to function. Injection well was not operating this day. Lease road is stable. Operator has installed a lease road gate. Load lines were locked. Man door to the tank battery has a lock installed on it.

1321452143 2011-11-15 Operator has tripped the reconditioned Baker Lockset packer into the hole. The operator circulated the inhibitor bottoms up and the packer was set at 3304'. Tubing (2.375", R-3 4.75 lb) was set at 3300'. The operator pressured up the annulus to 780 PSI and tested pressure at 5 minute and 15 minutes with no drop in pressure. Passed the Mechanical Integrity Test (MIT). The work pit was lined and the pit fluids were pumped into a water truck after work was completed. On the previous service day it was believed that the lower section of perforated zone sanded in. The operator had circulated on the well to clean sand from perforations on 11/14/11. On that date they had 17' of open perforations. The operator had planned to attempt to clean additional sand out of the hole but felt they had enough perforations open to operate the well.
1325090983 2011-12-27 Annulus pressure was zero. The tubing pressure, when tested, was zero. There was a small amount of flow when testing for vacuum. There was a bull plug on the left hand side of shut off valve on annulus which was not tight. The shut off was threaded tight on the annulus. The fence around the wellhead, which was taken down when operator serviced the well, has not been repaired.
1335356617 2012-04-23 My inspection today found no violations. The annulus pressure on the well was 300 PSI. The injection pressure was 500 PSI. The well was not operating at the time of my inspection. The area around the wellhead, which had been disturbed by working over the well during the winter, has been graded. This area is barren at this time and it may need to be limed, fertilized and seeded if vegetation does not reestablish itself. The brine scald, found south of the tank battery dike during previous inspections, has been bio-remediated and vegetation has started to reestablish itself. The lease road gate was locked at the time of inspection.

1364750994 2013-03-29 My inspection found the well was not being operated at the time I was on location. Tubing pressure was at 0 psi and the 4.5" was at 575 psi. The pressures were recorded using a Division of Oil and Gas guage. The tank battery was surrounded by a proper dike and a chain link fence. The tank battery consisted of six 210 BBL tanks and three 300 BBL frac tanks. The building that houses the pumping equipment was inside the dike as well. The proper identification was located on the tank battery. The lease road was in stable condition. No violations found at time of inspection.

1480667397 2012-07-17 I inspected the site to check the status of the annulus pressure. Inspection on 06/29/12 found the annulus pressure to be zero. Inspection on 07/11/12 found the annulus pressure to be 750psi. On this day the annulus pressure was 720psi. I call J.R. the well tender and left a message requesting that he releive the pressure on the annulus to 250psi. If annulus pressure increase, further assesment of the well will be needed. I also asked him to call me and let me know if he charged the annulus, this would explain the increase in pressure. The loading pad was free of water and brine. The lease road gate was locked. Bull plug has been placed in the 1/4 inch valve.

2059367429 2011-10-18 Annulus and injection pressures were over the limit of the 1000 PSI ODNR issue gauge. The needle gauge that the operator had on site read over 1100 PSI. Spoke to JR, the well tender who was on site. He stated that 150 barrels of fresh water had been put down the hole and that caused the over pressure of the injection. 1. Injection well is to be shut down and pressure relieved. 2. Once annulus has equalized with the tubing an MIT test must be completed and passed before injection well can go back into production. Spoke to JR on 10/20/11. He stated that the operator is going to get a service rig on site, pull tubing, replace tubing and inspect packer. Inspector will need to witness activities. 9-3-07(D)
