1392388956 2014-02-14 I am on site today to plug this well with Glenn Davis Well Service and Formation Cementing Service. Davis has 2-3/8" inch diameter tubing @ 5680 ft. Formation will set a 200 ft plug from 5680 ft to 5480 ft c/w 25 sks. Davis pulled 49 jts of tubing up to where the 4-1/2' inch diameter casing has been perfed at 4100 ft per Inspector Grimm. Set plug # 2 from 4100 ft to 3900 ft c/w 59 sks. Then all of the tubing will be pulled and Davis will try and rip the 4-1/2 " inch diameter casing and pull it all out and run back in with tubing to finish plugging the well to surface.
1400600231 2014-05-20 On the day of my inspection accompanied by Intern Austin Gutridge, I witnessed that the site did not pass final restoration according to 1509.072B. I made call to Nighthawk Holdings at 330-674-5391 and spoke with Rob Hunter and stated that the equipment needs to be removed and he stated that it would be done this week and would notify me when done so that a follow up inspection could be completed.
