1107498199 2010-12-21 Complaint from Mr. McIntire across the road that brine trucks leaving valves open as driving down the road - has pictures to give to AG's from last Thurs, he says. As I drove the roads to facilty, saw no sign of melted snow from brine, no damage at all. Talked to Jay at site and asked if they were leaving valves open - he did not believe so. Got a complaint last week from OEPA from Mr. McIntire that dumping brine on roads - called and found out they were having to transfer brine from big trucks to short trucks at end of road - possibly a small amount from that procedure but today saw no kill area or melted snow at end of road where occurred. Explained to Mr. McIntire that insugnificant amounts were not regulated, that with a permit companies were allowed to spread brine on roads. Told him I was concerned over 10 bbl being dumped but not a gallon at a time. He got very angry, continues to complain about the facility across the road from the house he rents. I saw no problem this day.
1123697678 2011-05-24 Minor reclamation still needs completed on site due to recent tubing/packer work. The plastic fence is still up around where pit was located. The site needs final grading and additional seeding, especially where drill pit and other excavation occurred. There is a frac tank still on location, near the well, which should be removed. 11/21/11 - Frac tank has been removed and the site has been reclaimed from recent work-over on the well. All violations RESOLVED. NONEC
1130592022 2005-06-15 Inspection this day found location had been reclaimed, has been seeded and mulched, still small amount of black plastic from the pit didn't get buried as did several of the plastic collar protectors from the casing that was run. Will ask them to remove. Well producing, current chart in meter, GPS'd locations at earlier date.

1190896581 2007-09-28 Inspection this day required to run packer into new disposal well. New Baker "J" or tension packer run into hole on new 2 3/8" tubing to 6926'. Baker man on location to supervise running of packer. Water well in use - landowner estimated it 40' deep, at GPS N39.43571, W81.31373. Also water well further away at GPS N39.43188, W81.31466, in use. Tanks will be set on top of hill, about 150' from well - further downhill from well about 200' is a low, usually swampy drainage area coming off the rolling hills. Will try and take a couple pictures. This day spent running tubing and packer to 6926', then loaded the tubing slowly with fresh water but still too much air in it at 6:30 PM to complete the pressure test this day. 9/28 - Returned to find air out of tubing, ran pressure up to 1775# with pump truck, after 15 min. was at 1650#. Passed. Next sheared pins off bottom of packer with 2700#, circulated to make sure packer open, then filled with packer inhibitor. Set packer at 6926', tested annulurs to 525# for 15 min. No drop in pressure. Completed volume/rate tests that showed at 1 bbl/min, needed 700#, 1/4 bbl/min, needed 50#. All were satisfied.

Note: Setting depth of new packer is 6891' to center of element and overall length of string is 6894.'

Will pressure test tomorrow.

Ben called later - says Tom has given permission to use.

227923874 2010-11-30 Service rig runs in remaining tubing in hole to depth of 6829'. Baker Hughes man on location to set Baker A-1 Lok-Set Retrievable packer and Universal used to pressure up well. Pump truck took pressure up to 1690, tested their lines, then opened valves to pressure up on well. Immediately fell to 200#, indicating a leak in tubing. Baker set packer, connected to 4.5" casing to see if tubing would flow and it did, also confirming hole in tubing. While pump truck on location, pulled tubing 10 joints at a time, testing pressure after each 10 joints to try and isolate joint with hole. Pulled 50 joints this day without finding problem. Will continue tomorrow.
265561816 2009-09-11 OEPA Deavers called to say received a call on 9/10/09 about truck "dumping brine over the hill off the side of road, oil on the ground, and man's dog sick after drinking the dumped water in the creek". Found area about 3' x 10' showing brine kill, but did not find it to go all the way downhill into the creek, green grass/weeds between the 2 areas. George McIntyre, the complaints came down the road to show me - I agree a small amount of brine was released from truck, small oil spot in middle of road (where truck sat) about 3" x 6" and could be transmission fluid/oil from truck. Says brine was dumped 9/2/09 from an orange and white truck and talked with Grant, monitor at well, to try and determine which truck did it and why. Grant said he would review records and figure out which driver it was and talk to him. Virco has had trouble with this man for many months - has already gone to court once with Virco winning, man claims to be disabled veteran - said his dog still sick from drinking brine - told him to take him to the veterinarian.

Location still needs reclamation work - plastic fence still up where the work-over pit was and ground below the facility needs more grass seed - only growing sporadically. Good ID's, unloading pad looks clean and inside dike area mostly dry.

711433922 2012-02-10 My inspection this day due to quarterly requirements. The pump was running and pressures were 1500# on the tubing, 0# on annulus. Two trucks with brine numbers were unloading while I was there. While inspecting the steel flow line from the pump building to the well, found a badly rusted and rotted steel connection - advised them to get it changed out in next few days. Unloading pad clean, good identification, pumphouse clean also. The weather has washed out some of the black plastic used to line the pit when drilled - told them they needed to remove all that plastic - they were instructed to bury it deeply when reclamation was completed. 2/14/12 - Upon inspection, I found the 2" steel line in rusted condition has been replaced with new line. Operator will take propane torch and burn up the plastic found on the ground.
