The well is in production and there is 36 PSI on the annulus. Tank battery is fenced properly, the dike is in good repair, equipment spacing is correct and the man doors are locked. The separator is iced over. Contacted Warren Shoenfelt and informed him of the icing issue.

-1959939857 2009-02-25 Rig moving on site at the time of inspection. Pits are in good repair. Clay sides, small amount of water in the bottom of the pits, less than 2 inches in depth. No water seeping into the pits was noted during inspection. Reminded Hugh Palmer that sides of the pits need to be sloped, not dug near vertical. Drainage controls are in place north of the site. Safety fence was up on the west side of the pits. Permit ID sign was up. Permit was in the dog house.
1236108805 2009-03-03 Operator has completed drilling well. Well was TD at 3799' No shows of gas during drilling, per driller. Top of the Lockport was at 3100'. Operator ran 210 sacks of class A cement during the cementing of the long string. Good circulation during cement job. Pits were in good repair. Site was in good repair.

2015822908 2009-02-26 On site for surface cement job. Operator had ran pipe before I was on site. No loss of circulation during drilling. Fine black film was noted in the pit and coming up during circulation. Rig hand and Eric Poulson gathered a sample, sample had no odor. There was not enough to gather a sample for testing. No oil smell to substance, it appeared to be shale fines. Eric stated that they get this substance about every other well, in the area. Cement job went well. Cement held at surface.
52706841 2013-05-28 My inspection found the well to be equipped with a plunger lift unit. The annulus valve is open to a pop off valve. The tank battery and well are in good repair and surrounded by a chain link fence. The tank battery consists of a 100 BBL production tank, a 50 BBL poly brine tank, and a mechanical separator. The vent stack on the production tank is plumbed into a 5 gallon charcoal filter. The vent stack is then plumbed up and topped with a spark arrestor. The proper identification is located on the tank battery. The lease road is in stable condition.
