-1311250936 2014-08-01 I accompanied Doug Bennett during the inspection of this well. We were onsite to witness the surface casing being cemented.
-1927814505 2014-06-02 On the day of my inspection I witnessed Precision Rathole Inc. drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. O-Tex Pumping LLC was on site and gained circulation with 2bbls of fresh water, 15bbls of gel and 10bbls of fresh water. O-Tex cemented pipe with 310 sacks of class H cement. Good returns of 13bbls and cement stayed surface.
-804094069 2014-08-12 On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I received a phone call from land owner John Dolin about something flowing out in front of the pad and he was concerned about his water well getting contaminated. Upon arrival we found that the sumps were being discharged and being monitored hourly for pH. Completed walk around with Colden Parker Safety Specialist and Tom Bauer Environmental Specialist and checked water with Quantabs which read 96ppm. Instructed Tom that we also needed the chlorides checked along with the pH. We then met with Mr. Dolin and told him of our findings.
1406897328 2014-08-01 On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Nomac Drilling Rig #137 drilled a 17.5 inch diameter hole to the depth of 400 feet and Wyoming casing crew ran 400 feet of 13.375 inch 54.5#/ft. J-55 Surface Casing. O-Tex Pumping LLC was on site and gained circulation with 5bbls of fresh water, 30bbls of gel, 10bbls of fresh water and cemented casing with 630 sacks of class H cement 15.6ppg, 1.2cuft/sk. with 2%CaCl, 0.4% C41P and ?#/sk. of Cello flake. Displaced cement with 56bbls of fresh water had good cement returns of 18bbls, plug was landed at 250psi and pressured to 800psi for 5 minutes and the floats held. O-Tex Chart recorder failed to record entire job.
1782923336 2014-08-14 On the day of my inspection I witnessed that Nomac Drilling Rig#137 had cemented the 5.5 inch production string and was preparing to rig down and move to Harrison County.
