Also present a GP Unit and a sand trap. The well was equipped with a plunger lift system with valves in the open position. Gauges present read 400 pounds per square inch on the casing and 200 pounds per square inch on the tubing. Annular pressure monitoring was present along with proper identification and production records being current. |
0428367103 | 2019-04-29 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the wells remained temporarily abandon with downhole plugs and caged at surface. A workover rig was rigging up on location to drill out frac plugs and install production tubing within the 2 wells that were recently drilled on the pad. Sound walls were being removed from location. |
0449552695 | 2014-08-12 | Drill / Deepen / Reopen | No Violations | On the date of inspection I accompanied ODNR Inspector Doug Bennett to respond to a complaint from a landowner. Refer to Doug Bennetts report for further details. |
0594778067 | 2018-04-19 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. The production equipment consisted of ten 400 barrel tanks, a vapor recovery tower, two compressor units, two horizontal heater treaters, an oil stabilizer unit, gas production unit, sand trap, and a gas combustor. |
0804094069 | 2014-08-12 | Drill / Deepen / Reopen | No Violations | On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I received a phone call from land owner John Dolin about something flowing out in front of the pad and he was concerned about his water well getting contaminated. Upon arrival we found that the sumps were being discharged and being monitored hourly for pH. Completed walk around with Colden Parker Safety Specialist and Tom Bauer Environmental Specialist and checked water with Quantabs which read 96ppm. Instructed Tom that we also needed the chlorides checked along with the pH. We then met with Mr. Dolin and told him of our findings. |

1412369194 | 2016-09-06 | Production Wells | No Violations | On the day of my inspection I witnessed that the well was equipped with a plunger lift system with valves in the open position. Gauges present read 500 pounds per square inch on the casing and 300 pounds per square inch on the tubing. Production equipment included a sand trap, a GP Unit, a line heater, a compressor, a vapor recovery system and 2 disconnected NGL tanks along with 10-500 barrel tanks shared with the 2H and the 4H within containment. |
1460044314 | 2016-04-07 | Production Wells | No Violations | On the day of my inspection following up Complaint ID 5295, arriving onsite at 0910 and monitored noise levels until 1000 revealing normal levels. Ascent Resources has completed installation of a sound wall on the Northeast side of the well pad with continuing monitoring of the noise level coming from the pad. I placed call to Ascent Resources EHS Supervisor Bob Adams and requested information on the Lake Pad for the last two weeks. Mr. Adams stated that normal production was occurring at the pad during the time in question. I then placed call to Ascent Production Supervisor Darren Rader and he stated that the temporary sound barrier around the combustor was damaged in the wind storm two weeks ago and that he was having the base engineered to prevent future damage before re-installation. Mr. Rader stated that he would try to have it back in place next week. The well was equipped with a plunger lift system and valves in the open position. Gauges present read 1400psi on the casing and 700psi on the tubing. Production equipment included a sand trap, a GP Unit, a line heater, an oil stabilizer unit, a compressor, a vapor recovery system, a gas combustor and 2-NGL tanks along with 10-500bbl tanks shared with the 2H and the 4H within containment. |
