Mote: Sulfur odor detected at wellhead, suspect hole in casing and possibly H2S may be present. Talked to company rep 1/12/11 concerning outstanding violations at well he stated that they would plug/plug back this well later on in the year.

The well is not capable of controlling the flow of gas and oil. The well is not equipped in a manner that would allow it to be produced and there are no pipelines attached to the well.

-280445812 2014-01-16 My inspection today finds the 4.5" casing has still not been repaired. The well is open to atmosphere and a small amount of gas is escaping the well. I noticed a slight sulfur smell. I will contact the company about repairing the well.

325656336 2014-02-28 I called Mr. John Snyder of Stonebridge Operating at 11:17 am. I discussed the outstanding violations against the well with Mr. Snyder. Mr. Snyder stated that he was planning to get a permit to plug the well. He also stated that the proper identification would be in place by 3/7/14.

491539565 2013-07-11 I called and spoke to Mr. John Snyder. I told him that the plug back permit for the well would be expiring soon and that something needed to be done with the well. Mr. Snyder stated that he intended to begin work on the well in the next two months weather permitting.

I told Mr. Snyder that due to the fact the well has been out of compliance for eight years and the casing has broken off at the surface, the company must make repairs or plug the well immediately.
