There was a light gas odor present but ODNR Honeywell PhD6 meter showed zero H2S & LEL present at the time of inspection. There was a 1 foot piece of 4.5 inch casing reduced to 2 inches lying beside the wellhead and no flowline or production equipment was present at the time of inspection. Identification was present and production records were current and up to date showing production in 2013. | |

Formation Cementing spotted a 57 sack class A cement plug at the bottom of the tubing with 10.3 barrels of displacement. Matheny Well Service pulled the tubing up to 1714. Formation spotted a 200 class A plug. The service company pulled the tubing out and ran wire line tools in to tag the plug and found the plug slipped down hole nearly fifty feet. The rig crew then ran tubing back down to the top of the prior plug and Formation spotted a 100 class A 1% gel plug at 15.2 pounds per gallon. The tubing was then pulled up to 200 from surface.
