-2066222449 2013-10-22 I witnessed Nick's Well Plugging plug the well for Petrox, Inc. Nick bull headed 25 sacks of Class A cement for Plug #1. The cement was displace with 60 bbls of salt water. Plug #1 was set from 3,716' to 4,002'. The operator pressured up to 900 psi and held it for 30 minutes. Next, the 4.5" casing was ripped at 2,760' and 8 joints were removed. Nick established circulation and pumped 86 sacks of Class A cement. The cement was displaced with 36 bbls of fresh water. Plug #2 was set from 2,172' to 2,472' to cover the top of the Big Lime. The operator pull 61 joints. Nick established circulation and pumped 139 bbls of Class A cement. I witnessed a good cement returned to surface. Plug #3 was set from 450' to surface. The operator pulled the remaining 13 joints and the cement was topped off. Cement was static at surface after 20 minutes. A total of 82 joints of 4.5" casing was removed from the well.
1842110072 2014-07-30 I was on location for a final restoration. I found during my inspection all of the production equipment removed. The site was graded. Vegetation was well established. The pits were filled and closed. All debris was removed. The lease road was being used to service an existing well. Final restoration passed.
