Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
2171364 2013-05-31 No Violations Noted 20" cond set. Need site sign.
2180077 2013-06-24 No Violations Noted The main intent of my inspection was to view the dewatering of solids during top-hole drilling operations. On 6/26/13 Carrizo submitted a request for an alternative waste management practice (OG-71) for this process to the Department. The system was running during the inspection. The process invloves a belt press filter system and a mixing/conditioning tank with the additon of a polymer to aid in the seperation of solids and liquids in the drill cuttings as they are blown out of the wellbore. All solid material generated from this process and temporarily held in roll-off containers onsite, will be disposed of at Alliance Landfill in Taylor, PA. The liquid material will be held onsite in frac tanks after the treatment process to be either reused during well drilling or transported offsite to another Well Pad to be used during completions. The OG-71 template contained a description of the process, an illustration of the operation and MSD sheets for Hyperfloc CE 800, which is the polymer utilized in the treatment process.

2332360 2014-12-12 No Violations Noted A Consolidated wellhead Service Rig was on the location in the process of cleaning out and setting kill plugs for the Plushanski West 2H well according to Mr. Robichaux. Freshwater and production fluid transported to the site from permitted wells on other Carrizo locations is used in the process of cleaning out the well. My meter indicated over the limit conductivity readings in water on containment next to the frac tank battery containment berm. Mr. Robichaux indicated residuals from a fluid hose was spilled on the containment as the hose was broke down from the frac tank. He will have a vacuum truck remove the fluid off containment. Production fluid is stored in the frac tanks and freshwater is stored in the frac tanks covered with plastic sheeting. A heating pump is used to prevent the water from freezing inside the tanks.I observed (2) small areas of staining in the snow on containment below the heating unit. I also observed a small pool of diesel fuel in containment on the access road.
