All pits used for drilling shall be closed two months after completion of the well. A two inch line was being run from the 210 barrel salt water tank into the pit. At the time of inspection a small amount of fluid was draining from the line and samples were taken. A high range Quan-tab test strip was used to test for Chloride levels in the water coming diectly fro the line and 6797+ parts per million Chlorides were noted. The line running from the salt water tanks to the pit shall immediately be disconnected and removed. Field Visit or Inspection

-1213877429 2011-09-13 Administer BOP test for Bernard 167. 500 psi required, hold pressure and loose no more than 5% pressure over 15 minuets. Three tests administered and each failed. BOP not a permit condition, owner running BOP voluntarily, so drilling will continue. Blowline only 30 feet, so I suggested lengthening it. 10 extra feet there on site, so line was extended.
1315845200 2011-09-12 Drilling status check. Currently at 270 feet, 12 1/4 surface casing borehole. Pit liner in good shape and pit of sufficient size to hold anticipated water. Mud pump also on site.
1315939254 2011-09-13 Met with J. Scott to administer fresh water surface casing test. 365 psi required to hold for 15 minuets. Failed test 3 times.
1316125020 2011-09-15 Witnessed Longstring cement job. 85 sacks of class A with 200 lb gel, 25 lb flake, and 2 qt. polymer. 7 inch casing inside 8 3/4 borehole. Note: well is permited to 1900, TD was 1999. Spoke with Steve Opritza and allowed company to continue.
1329504836 2012-02-17 The pit is being emptied by the operator.

-1853011790 2011-09-13 Witnessed surface cement job. 727 feet of new 9 5/8 cemented in 740 feet of 12 1/4 borehole. 100 sacks of 1 % gel ran ahead and 180 sacks of neat ran behind. 400 lb gel, 700 lb accelerator, 180lb LCM, and 2 qt. polymer. Circulation established, good cement returns, cement level stayed at top.
193578895 2012-03-02 The pits have been closed. The dozers are being moved to the next restoration.
194341475 2012-02-13 The pit is still open.
-1996475733 2012-01-23 Drill pit is open. Pit liner has fell into pit in multiple places. A 2 inch black plastic line has been installed between the drill pit and a 100 barrel steel production tank drain. The tank drain is equipped with a 2 inch valve, a 2 inch 90 degree elbow, a 2 inch clear plastic nipple, 2 inch steel hammer union connected to the 2 inch black plastic line that extends to the drill pit. Oil and Gas Supervisor Gene Chini is scheduled to see well site on 1-24-12.
-2064669775 2012-06-11 Area graded, seeded and mulcted, the pits are closed, and there is no erosion on site.
2105829179 2012-04-27 Area has not been graded and vegetative cover has not been established. Area shall be graded and vegetative cover shall be established in order to prevent erosion.
-356720151 2011-09-19 Checking on pit level. Close to top due to rain, spoke with Tom Stone of Mountaineer State and will drain soon.
39964713 2011-09-07 Predrilling site review with Tom Stone of Mountaineer State Oper LLC. Well appx 150 feet from driveway and 100 feet from barn.
745062078 2012-01-24 Inspected well site with Josh Garland MRI and Gene Chini Oil and Gas Supervisor. Drill pit is open. Water was discharging from the 2 inch line into the pit. A sample was taken and sent to the lab for analysis. A High Range Quantab was placed in the discharge water and the brine exceeded 6797 parts per million chloride. Informed Tom Stone and John Kent of Mountaineer to stop discharging brine into the pit and to disconnect the 2 inch line. Also told Tom and John to pump out the pit and take the fluids to an approved salt water injection well for disposal. Then close the pit.
-878328239 2012-02-02 The Frac Tank has been removed, the pits are still open, a water knock-out has been installed, and a second saltwater tank has been installed.
