INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1209112638 2014-01-06 Checked on well because Columbus had asked me to check on Annular Disposal status of Parker #1 well on same lease. The water from the Parker #1 was being transfer from well #1 to the tanks associated with the Parker #2. Surface casing head visible, water being stored in a 150 Barrel steel tank and disposed of in a liquid tight system. No noted violations at site.
1268918488 2010-03-17 Pits are open and full of oil and fluids. A steel 210 bbl tank is on location and connected to production equipment and to the well. A 1 1/2 black plastic line is connected to the tank drain and has been ran to the pit. Well is equipped and capable of gas production. Well is equipped with a pump jack, rods, tubing, electric motor and electric service. Electric service line is ran on top of the ground from the pole to the well. Some welding has been done to the wellhead. A test rig on the annulus with a gauge and a quick connect suggests testing for AD. Inspector has not been notified of a AD test. No Identification on well or tank. Spud date 8-27-2009. 1509.072 (A) Within 5 months pits are to be filled in. Fluids in pit must be disposed of in an injection well. Well needs Identification Per 1501:9-9-05(A)(10) All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank, as to owner, lease name, well number, permit number, county, and an emergency telephone number. The electric service line is on top of the ground. Bury the line or run line on poles to the well.
1421237739 2015-01-13 A mechanical integrity test was attempted by the company to retain their AD status on the Parker #2. Two attempts were made to administer the test but the well was unable to hold the required pressure due to minor leaks in fittings at the surface.
1427141861 2015-03-23 Attempted to administer annular disposal test but annulus did not have sufficient pressure. Will return later this afternoon to administer test.
-1755793641 2009-11-06 Status check to see if ready for AD test. Rig is still on location and operator is preparing to pull rods and tubing out due to sand build up. They will call when ready for AD test.
1926349846 2015-03-23 Attempted to administer the 5 year positive differential annular disposal test. Well was unable to hold the required pressure for the hour long testing period.
-1985234387 2013-02-19 Well equipped with a pumping unit and an electric motor. The valves on the production casing and the tubing were open. The tank battery consists of a 210 Barrel steel tank plumbed up for annular disposal and a separator. Water was seen running out of the dike drain and was field tested for Chlorides using a Quan-tab test strip. It was found to have less than the EPA drinking water standards for Chlorides levels.
-776914709 2013-06-06 Follow up to 1268918488. No violations at time of inspection. Well is plumbed up for Annular disposal. Pumping unit running at time of inspection.
