-1123150468 2010-09-17 Attempts made to speak with Mr. Doug Kithen by telephone regarding plugging requirements were unsuccessful. Therefore, Mr. Ron McBride with Chieftain Energy was advised of requirements at 9:30 a.m.
-1141338772 2008-02-05 A follow-up inspection revealed that the leak from the flow line has not been repaired. The impacted area from this leak is approximately a 10x 15 area. Quantab (high range) reading reached a maximum unit of 10, which greatly exceeds the US EPA Secondary Drinking Water Standard of 250 milligrams per liter. Doug Kitchen was instructed to remove this soil from the site.
1202308848 2008-01-28 A follow-up inspection revealed non-compliance. The well is still idle and no identification was observed. Mr. Doug Kitchen states that the well has been pumping, but had to repair the production line. The broken line was located approximately 150 feet behind the tank battery. At the time of the inspection, the production line (black plastic) was found to be leaking. The leak is a slow constant drip. Mr. Kitchen was informed by telephone to repair this line immediately and remove contamination and pollution.

Dike does not meet requirements, not large enough. Dike size is 30x40x1, will hold 195 bbls of fluid, dike has 3-100bbl, 1-200bbl tanks. NEED FULL ID AT THE WELL AND TANK BATTERY, CLEAN UP ALL OIL, AND OILY SOIL AT WELL AND DIKE OF TANK BATTERY. BUILD A DIKE TO HOLD AT LEAST 220BBLS OF FLUID.

192803336 2008-04-02 A complaint was received alleging that oil has been released from a broken flowline. The owner, Chieftain Energy was contacted and informed of the complaint. An inspection revealed that the oil release was the result of a ruptured flowline. Contamination and pollution was not observed outside the dike. A valve had been closed on a frac tank. As a result, the plastic line ruptured due to pressure. Chieftain Energy had previously stated that this tank was not being utilized. The company was observed on-site performing cleanup. They were instructed to remove oily soil and evaluate 100bbl tank that may contain a small leak underneath and rebuild dike. (1) 100bbl tank was found to be only 40ft from the road. The company states that they will not be utilizing this tank. In addition, the area is limited due a right of way located behind the tank battery.

2026373130 2008-03-18 A follow-up inspection was conducted, in response to landowner complaint alleging that a flow line is leaking. During the inspection, it was noted that a black plastic flow line had separated inside the dike. Small amount of C&P was observed. The line appeared to be routed to the top of a 100 bbl tank. Chieftain Energy was contacted by telephone and stated that the line was not being utilized, but would perform repair and cleanup.

An inspection revealed that the well does not appear to be producing and identification is incomplete. Production records indicate that the last year of production was 1996. Landowner has not received any monies for several years. No electric for the motor on the well. The well owner, Mr. Doug Kitchen, states that he has not been producing this well and to send him a NOV.
