-1203266279 | 2013-10-22 | Received phone call from Donnie from Knox Energy. He stated that they first tried to rip at 3800 ft, and the first telegraph they got was at 3000 ft. After consulting with Inspector Pat Shreve, I told them that they could rip there, pull the pipe, and run tubing back down to that depth and set a rip plug there to cover up the Big Lime.
-1315370326 | 2013-10-25 | Witnessed the setting of the Lime plug, Berea plug, the bottom of surface plug and the surface plug. The Lime plug circulated. The others did not circulate fluid but pressure was observed on the backside during pumping. A bridge was set for the surface plug. Fluid level was 6.5 seconds down. Well was cemented from there to surface.
1315387920 | 2011-09-06 | Inspection this day due to complaint by landowner, Mr. Mike Whited. Says well has not produced in long time and black plastic line not buried, as required in lease, on his property. Found well standing idle, cedar tree growing up into it and could barely see well. Wellhead, casing rotted thru and sitting at angle - will need to be fixed to produce but found no odor - well not leaking. Valves shut - last annual production reported for 2010 but this is mistake - not possible well produced in 2010 and landowner verifies this. Says he has not received royalties since he moved in @2004. No ID's at well - required. Tanks have good ID's, chart in meter has no date, says it is for Yaeger #1, #2. Well needs put in production, ID's posted.
1334653183 | 2011-11-07 | Inspection this day found Broad Street's service rig set up on well. They are getting a water truck to dump water down tubing to see if well will circulate. It may have a hole in casing as the other Yaeger well does. Later in day the pump truck had to leaveand they did not get well pressure tested.
-1639818049 | 2013-10-15 | It was reported to me that that there was a packer on the end of the tubing. It got caught on a hole in the casing and the tubing had to be shot off. During my visit the service rig crew was attempting to beat the packer down to the bottom of the well using the stem and jars. They were at a depth of 2100 ft.

-1912504316 | 2013-10-17 | Andy Thomas called and the Rig was beating on packer with jars to drive to bottom.
633202580 | 2013-10-15 | I received a phone call from Mr. Ed Butler at 12:37 PM. He stated that they could not get the packer on this well to move to the bottom of the hole. They were at about 2700 ft. I told them that they needed to get to the bottom of the hole in order to set an adequate bottom hole plug. I suggested that the packer may have to be milled out.
-676661496 | 2013-10-10 | Crew was pulling tubing at the time of inspection. It was being difficult to pull. Spoke with Mr. Kenny O'Brian from Broad Street and Donnie from Knox Energy. We discussed plugging procedure.
-694666386 | 2004-12-27 | Inspection this day found well shared with 8491, being metered together with current chart, not turning any production in for it but way up behind house and can't get to - may be house gas also, ID's good on 100 bbl tank, fair to good dike
-711616392 | 2013-10-22 | Witnessed the bottom hole plug being set. The bottom of the tubing was set at 4994 ft. A 30 sack plug was set at this depth. |
