-1286411547 | 2014-04-10 | I performed a follow-up inspection concerning the clean-up operations for the oil spill. My inspection found all of the impacted grass and soil removed. The 210 barrel production tank was cleaned of all oil. Peat moss was spread over the entire impacted area. The well tender was on location and stated a new identification sign had been ordered. I will follow-up on the identification sign. Photos were taken. | ||||
1336614562 | 2014-02-28 | I checked this well to determine if the company is in compliance with chiefs order # 2014-43 . I found the tank battery is equipped with 1 100 bbl , 1 210 bbl steel tanks and 1 separator . The 100 bbl tank was empty and the 210 bbl tank was 1/3 full of fluid . The date marked on chart on gas meter was 2-26 , the year was not indicated . The identification sign posted on the 210 bbl tank was mostly ripped off and was not legible which is in violation of 1501: 9-9-05 , A-10 of the Ohio Administrative Rule . The valve from the flow line to the separator was closed . The earthen dike protecting the tank battery is not of sufficient size to contain the amount of fluid which could be stored in the production tanks , which is in violation of 1501:9-9-05 , A-9 of the Ohio Administrative Rule . At the well location I found the well is equipped with a pumping unit , the pumping unit was not in operation at the time of my inspection . There is a domestic hook-up from the production casing with the valve closed . There is a 6' wood fence surrounding the pumping unit . NOTE : This well is located on the Bristolwood Golf Course . This well was not in production at the time of my inspection . | ||||
1378992713 | 2013-09-06 | I received a inspection request from the Bristolwood Golf Course. My inspection found the well not in production. The well was equipped with a pumping unit with all the valves to the flow line closed. The well was surrounded by a wooden fence with half of the panels either missing or lying on the ground. I noticed a 4' diameter greasy black stain and an 18 diameter muddy area on the grass located approximately 6' southeast from the pump unit. The head maintenance employee, Leroy, claimed a Big Sky crew had been on location working on the well the entire week (9/3/13 to 9/5/13). The tank battery contained a 100 barrel production tank, a 50 barrel brine tank, and a separator. The dike was slightly weathered; however, it meets containment requirements. There was no identification sign posted. I called Valerie Dunne of Well Completion Services at 440-858-2343. Valerie stated she would dispatch Ryan, their well tender, to meet me on location. Once Ryan arrived at the golf course, he claimed the well was shut-in to replace the rods, which caused the greasy black stain on the grass. Also, he said the muddy area was a flow line leak. Big Sky pumped fresh water from the tank battery through the flow line to locate the leak. I instructed Ryan to begin repairs on the flow line immediately, post the proper identification sign, and replace the wood fence panels. At 11:20am, I received a call from Bob Barr, Jr. He stated Big Sky would start repairs on the flow line by 9/10/13. I took photos of the well location. | ||||
1546218246 | 2013-09-11 | My inspection found the flow line repaired, soil compacted, seeded, and straw spread. I took photos of the repair. | ||||
1561790158 | 2013-10-01 | I performed a follow up inspection of the brine spill from the tank battery. My inspection found lime spread throughout the tank battery and lease road. I noticed a section of grass next to the lease road southwest of the tank battery that was brine scolded. The approximate area was 2' wide by 15' long. According to ODNR Inspector Kevin Perry, Bob Barr, Jr. told Kevin that Big Sky would till up the brine scolded area next to the lease road and re-seed. I called Bob Barr, Jr. at 740-624-0434 and left a message to finish tilling the affected grass area, then seed. | ||||
-1660003801 | 2013-09-24 | I was on location today in reference to complaint #4467. Bob Barr Jr. was on location with K.D.A. vac truck. The 100 BBL brine tank had rusted through on the bottom of the tank. The brine brine that escaped through the dike had pooled up in the lease road and some made it into the grass. Bob said that he has a crew on the way over to the location. The are going to till up the affected area, mix in soil and lime, and then seed and straw the area. The 100 BBL tank is going to be removed from the location. I informed him that he needs identification on the location. The fence around the pump jack needs repaired. The stuffing box seals need to be repaired and the area around the wellhead needs cleaned up. | ||||

-2007848716 | 2014-04-14 | I responded to a call about an oil spill at the tank battery. My inspection found the 210 barrel production tank covered with oil. There was an approximately 20 gallons of oil lying around the 210 barrel production tank and between the production tank and 100 barrel brine tank. All of the oil was contained inside the dike. I noticed the identification was ripped in half. Bobby Barr of Big Sky was on location. I instructed Bobby to suck up all of the oil, remove all of the impacted grass and soil, clean all of the oil off the production tank, and replace the identification sign. I questioned Bobby as to why the tank overflowed when the well was supposed to be shut-in per Chief's Order 2014-43. Bobby claimed the well was shut-in. Photos were taken. | ||||
-2040744135 | 2013-09-10 | My inspection found Bob Barr, Jr. and crew making repairs to the flow line next to the well. I noticed a 2 black poly flow line was exposed. The line was approximately 12 deep. Bob stated once the flow line was repaired; the soil would be compacted into place and seeded. I noticed no evidence of any impacted soil from the flow line leak. To prevent a safety hazard to the golfers, I told Bob further inspections of the well would continue until the fallen and missing wooden panels surrounding the pumping unit where repaired. | ||||

He stated Bob Barr Jr. was on location putting the pumping unit into production. He stated the golf course maintenance crew was video taping Bob performing the act. | ||||
-572123044 | 2014-06-04 | I received a call at 1:56pm from the Bristolwood Golf Course owner stating that Bob Barr Jr. was on location with three employees, a tanker truck and a pick-up truck operating the well. When I arrived at the golf course; Big Sky was not on location. I spoke to the owner; who showed me photos of Big Sky working at the well and tank battery. Big Sky's operation of the well was in non-compliance with Chief's Order 2014-43. | ||||
